• Full Time, Days. 36 hours a week.# BCH is seeking a Therapist who serves the needs of patients in the Center for Mind Body Medicine clinic. CMBM empowers patients with evidence-based, self-care tools for improving physical health and emotional well-being. Drawing on new theories in neuroscience, our treatment involves teaching you how to change long-stored memories in your brain#s neural pathways to help alleviate or significantly improve stress-related emotional and physical illnesses, chronic pain and hard-to-treat medical conditions.#The position requires the ability to promote the coordination, collaboration and continuity of care from admission through discharge promoting the appropriate level of care and coordination of insurance benefits.# The position necessitates leadership skills with the ability to direct and evaluate all aspects of patient care, and to assure that they are performed in a timely, effective and efficient manner.# Must have a working knowledge of managed care, its terminology, structure and process.# Excellent organizational skills and the ability to work and communicate well with others are critical aspects of this position. Responsibilities #Assist in admission and orientation of patients. #Review initial assessment(s) and assist in completion. #Conduct patient assessment evaluation and communication to the attending M.D. and treatment team. #Provide leadership in completion of the Master Treatment Plan and ongoing treatment goals. #Develop, coordinate and complete discharge planning. #Facilitate patient access to needed resources and aid in the development of alternatives as necessary to meet identified needs. #Meet with patients (weekly) to assess and evaluate status and discuss measurable treatment goals. #Actively involve patient and family in the planning and implementation of care as appropriate. #Maintain communication with the patient#s referring therapist, psychiatrist and/or primary care physician. #Management of patient milieu.
