Responsible for providing general psychological care and treatment to patients in medical facilities such as the clinic, health center, or public health agency.

Primary duties may include, but are not limited to:

  • Performs mental status examinations, therapy, and preventive health measures.

  • Reviews psychological diagnostic tests to identify and assess patient's clinical problems and health care needs.

  • Records physical findings, and formulates a plan and prognosis, based on the patient's condition.

  • Discusses case with the Physician and other health professionals to prepare a comprehensive patient care plan.

  • Submits the health care plan and goals of individual patients for periodic review and evaluation by the Physician.

  • Administers or recommends individual therapy or other forms of treatment.

  • Refers patients to the Physician for consultation or specialized health resources for treatment.

  • Requires an MS in Social Work, MS in Marriage and Family Therapy, or a PhD in Psychology; 5 years of experience; or any combination of education and experience, which would provide an equivalent background.

  • Current license as a Clinical Social Worker (LCSW), Marriage and Family Therapist (LMFT), or Psychologist in applicable state(s) required.

Social work interventions are focused on assessment, crisis stabilization, psycho-therapy, and discharge planning. At times, the social worker may provide service to different patient age groups, based on training and knowledge. SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS* 1.Provides initial and ongoing psychosocial assessments for patients having complex psychosocial needs and/or medical complex diagnosis. Ensures patient is progressing towards desired outcomes and identifies barriers that hinder effective patient care. * 2.Collaborates with the health care team and develops treatment goals and plans intervention that assist the patient and family to resolve psychosocial concerns that may arise as a result of medical and psychiatric illnesses, engagement in high risk social behaviors (i.e. substance abuse), limited finances and/or physical disabilities. 3.Provides crisis and supportive counseling to enhance the patient's and family's problem solving and coping skills. 4.Participates in the therapeutic process by conducting psychotherapy group for patients as well as family sessions needed based off of assessment. * 5.* Collaborates with community agencies to provide services to assist with housing, financial, transportation, psychosocial, educational, home health needs of patients/families post discharge. Ensures every patient has discharge appointments set at discharge and discharge planning is started at admission. 6.* Documents social work assessments, treatment goals, interventions, progression and discharge plans in the patient's medical record congruent with departmental policies. 7.* Maintains current knowledge of trends related to medical and psychiatric social work health care issues through attendance and active participation in continuing education and professional organizations. 8.Conduct intake screening assessments for the Emergency Room. Consult with the charge nurse and physician for disposition of patient. * THOSE SPECIFIC FUNCTIONS WITH AN (*) ARE ESSENTIAL FUNCTIONS CONSIDERED NECESSARY TO ACCOMPLISH THIS JOB. EXPERIENCE Required – At least the equivalent of 2 years of experience, which can include a graduate level practicum (20 hours per week for 2 semesters or 40 hours per week for 3 months). Experience must be focused in the area of inpatient hospitalization, partial hospitalization, crisis prevention, or outpatient practice. Preferred – Three years experience working directly with inpatient psychiatric care patients in addition to having medical social worker knowledge EDUCATION Required - Master's Degree from an accredited social work program. Must possess LCSW or other appropriate licensure. Preferred – LCSW who is approved by the state of Virginia to do supervision for unlicensed, masters prepared social workers.
