We are seeking qualified applicants to fill a full-time Intensive Care Coordinator Therapist II (Clinical Services Caseworker) position in the New Horizons Intensive Care Coordination Program.

This position qualifies for a hiring bonus of $5,000. This bonus will be paid in two equal installments as funds permit. The first installment of $2,500 is payable upon the County's first regular pay date following your position start and the final $2,500 installment will be payable on the pay date following the completion of probation.

This is a SAMHSA Grant funded position that goes through 9/29/2027.

About This Role:

New Horizons Intensive Care Coordination (ICC) Program offers a team environment and flexible work hours.

In this position you will:

  • Provide leadership at all levels in the creation and maintenance of a positive and collaborative working environment where all ICC staff and enrolled youth and their families feel respected and supported.

  • Maintains a caseload of up to 10 ICC clients.

  • Provides effective Intensive Care Coordination utilizing the high-fidelity wraparound (HFW) framework service to youth and their families.

  • Utilizing the high-fidelity wraparound (HFW) framework, conducting comprehensive assessments, including ASAM Criteria and DLA-20, including formulating diagnoses and making treatment recommendations.

  • Developing and implementing ICC treatment plans to promote progress of client.

  • Timely clinical documentation into EHR that meets requisite agency, Medicaid, MCO and licensure standards.

  • Provides crisis intervention for clients, evaluating risk and need for hospitalization.

  • Facilitate youth and family team meetings. Ensure that youth and family voice and choice is present at all meetings.

  • Write, revise, and distribute team plans of care for each team meeting and FAPT reports as needed.

  • Documentation of all services, contacts and attempted contact with client, their family and collaterals.

  • Collaborating with other departments, professionals, and other stakeholders in the community (i.e., school staff, probation officers, psychiatrists, social workers, private providers, and residential treatment staff).

  • Maintaining healthy boundaries with clients and their families.

  • Attend all mandatory county and state trainings on ICC and High-Fidelity Wraparound, to include refresher trainings. Participate in individual and group ICC coaching, as required.

  • Carry an after-hours work phone and provide telephonic and, at times, face-to-face after-hours support to youth and families.

Minimum Requirements:

  • Master's Degree in psychology, social work, counseling, or related behavioral science field

  • Ability to demonstrate licensure eligibility for professional licensure as LCSW or LPC in the Commonwealth of Virginia

  • 1-2 years of experience providing mental health, substance abuse, and/or co-occurring services with youth.

Preferences:

  • Experience with and providing the High-Fidelity Wraparound Model and Intensive Care Coordination services.

  • Experience providing youth mental health, substance use and co-occurring case management services.

  • Demonstrated completion of training in the national model of High-Fidelity Wraparound.

  • Experience providing crisis intervention and risk management.

  • Experience collaborating with other human services system service providers.

  • Experience conducting youth mental health, substance use and co-occurring assessments, to include the ASAM Criteria, DLA-20, formulating diagnoses and developing treatment plans for varying populations and complex clients and families.

  • Experience working with and presenting cases to multidisciplinary teams.

  • Experience providing services in home and community-based settings.

  • Experience collaborating with other service providers.

Special Requirements:

  • Must be eligible for professional licensure as LPC or LCSW in VA.

  • Must complete training in the national model of High-Fidelity Wraparound as required for supervisors and management/administrators.

  • Must possess a valid driver's license.

  • Must pass DMV record check.

  • Must pass TB test.

  • Must pass and maintain first aid and CPR certification.

  • Offer of employment is contingent upon the candidate passing a fingerprint based National Criminal History Record Check.

  • Child Protective Services Requirements: Effective 1999, mandated by the Department of Behavioral Health and Developmental Services 12VA35-105-400 rules and Regulations for Licensing Providers by the Department of Behavioral Health and Development Services related to Criminal Registry Checks, a search of the registry of founded complaints of child abuse and neglected maintained by the Department of Social Services.

Work Schedule:

This full-time position, based in our Ferlazzo office, requires a 4 or 5-day work week that includes evening hours.

Starting Salary Range: $74,217 - $88,167
