GENERAL SUMMARY AND SCOPE
Under the direction of the assigned BH Director has responsibility for direct services to inpatients including diagnostic assessments (psychosocial assessments); individual, family, and group psychotherapy; comprehensive treatment planning including discharge planning and coordination with the outpatient Behavioral Health Department (as applicable); and crisis intervention as necessary. Responsible for complying with all the requirements of the performance improvement program. Supports the philosophy and the mission of the Behavioral Health Programs. Functions with a high degree of independence and professional skill.
ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES
Maintains documentation according to guidelines. Provides diagnostic assessment services including a comprehensive psychosocial, alcohol and other drug assessment and recommendations when appropriate.
Add this sentence to sentence above: Develops a comprehensive treatment plan, in conjunction with the treatment team process, with consumer input. Obtains consumer and other appropriate signatures on treatment plan and necessary release of information forms.
Provides individual, group and family counseling using evidence-based approaches for identified patient needs. If face-to-face family counseling cannot be arranged, conducts sessions via telephone with natural supports and patient with patient/guardian consent.
Documentation reflects the delivery of clinical services and patient progress. Maintains clinical and statistical records within departmental guidelines and in compliance with the standards developed by licensure agencies and third party payers.
Interfaces with referral sources, inpatient services, and other community agencies as appropriate to maintain continuity of care. Alerts outpatient staff of relevant information during the patient's stay to aid in patient recovery/relapse prevention.
Assists with discharge planning as appropriate to role in order to ensure appropriate utilization of resources. Coordinates with the Outpatient Behavioral Health Department for clients served by/being referred to that program as appropriate to help provide continuity of care and prevent relapse/untimely re-admission.
Provides the necessary information, chart corrections, to all performance improvement program inquiries within the appropriate time frames.
Keeps current in the field of behavioral health in order to recommend changes in current and future programming.
Actively participates in and promotes patient satisfaction and safety initiatives.
Regularly demonstrates the ability to serve patients of all ages and of all cultural backgrounds in the routine performance of duties.
KNOWLEDGE, SKILLS AND ABILITIES
Master's degree in a mental health related field, plus appropriate license as required in the Ohio Department of Mental Health rules and the Ohio Department of Alcohol and Drug Addiction Services certification standards.
CPR and First Aid certification within 6 months of hire.
Computer skills sufficient to learn and utilize the electronic medical record are a mandatory requirement for all direct service staff and supervisory staff.
Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
Punctual attendance at assigned work location is required.
Sensitive to the cultural needs of the individual and/or family served.
Ability to read and write in small letters and numbers.
Ability to effectively understand and communicate in both written and oral form.
