On a scheduled shift, assigned to work with consumers living at the Small Group Residential site in Cuyahoga County. Using Social learning & direct skills teaching, implements assigned responsibilities as reflected in the consumers personal plan & behavior plan: provides intensive community based & ensures consumer skills enhancement in two major areas; independent living skills includes hygiene & personal grooming, medication management with staff assisting residents in accurately self-administering medication through monitoring psychiatric symptom management, food preparation, domestic chores, health, nutrition, physical fitness, money management & budgeting & utilization of community resources; social skills includes leisure recreation, communication skills & appropriate behavioral skills, assesses & intervenes to stabilize potential situation & works with residents to de-escalate problem solve (potential) crisis situation, & as necessary, intervene to ensure the safety of all residents & staff; teaches residents in meal preparation &/or prepares meals; staff will also function as a role model for residents by behaving in a manner which is both professional & caring; facilitates & supports Mental Health Recovery for individuals with severe & persistent mental illness.

Works with residents & other professionals to help develop effective, goal-oriented individualized service plans & behavior plans to facilitated residents skill development: evaluations consumer progress & recommends service revisions to supervisor, case manager & consumer; works with the CSN project director & ACT team director to develop a service work plan, outlining the specific duties & responsibilities to the residential support workers contribution to the Small Group program; will participate in program development & direction under the guidelines of the program director; participates in all agency, program staff &/or resident meetings &/or committees as necessary to carry out agency programs & services; may have to transport clients to appointments, school, work & activities. Completes all required records, reports & other required documentation in a timely & accurate fashion & other duties as needed or assigned: documents all residents observation & progress, including changes in residents behavior & progress made towards individual goals; completes logs of consumer contacts which list dates, contacts & services provided; maintains the cleanliness of the facility on a daily basis; attends training opportunities to increase skills & knowledge of job relation techniques (i.e., professional, helpful, friendly voice & attitude), when greeting & interacting with visitors & the community at large in order to increase customer satisfaction; utilize Ohio Department of Mental Health & Addiction Services approved office productivity software intranet & Internet resources & information systems in an appropriate work related manner as specified in the NBH Personal Computer Usage Guidelines for Employees.

Formal education in arithmetic that includes addition & subtraction & in reading, writing & speaking common English vocabulary. In Department of Developmental Disabilities, must also possess a high school diploma or a certificate of high school equivalence pursuant to Chapter 5123:2-3-01(F) (1) (c).

For positions that require operation of motor vehicle to transport consumers to recreational, educational &/or occupational activities, a valid driver's license is required.

The position has LIMITED benefits. This is NOT a full-time position. Regular hours of duty in a two-week period are NOT guaranteed or consistent. If your education is outside the United States you will be required to submit an education evaluation showing how your education equates to the US standard. On an on-going basis, reviews all case files for assigned resident's & assures completion of documentation & notes resident's progress. Makes chart entries on the individual resident's development regarding the resident's Individual Habilitation Plan (IHP) programs, his/her behavior & any physical or behavioral problems displayed; maintains program logs, records & evaluation tools as directed. Trains residents in personal hygiene, grooming needs & activities of daily living (i.e.: feeding, toileting, grooming). Assists in the development & implementation of each resident's habilitation plan & scenario which are suited to the individual resident's needs & capabilities & which may include interventions for managing aggressive/violent behavior; assists in the evaluation of each resident's progress in his/her I.H.P. & scenarios. Participates in the evaluation of the resident & ascertainment of the resident's present level of functioning; the development of individual & group programming, & the training of residents according to specified guidelines. Mandatory hours including overtime is required of this position. Takes baseline information on specific resident behaviors as specified in the habilitation plans & scenarios. Assists professional staff in the development & implementation of in-service training. Communicates with individual residents in a manner indicative of a related atmosphere, conducts all physical interaction in a safe & gentle manner. Receives a report from the previous shift & makes walking rounds to account for the safety of all residents, carefully observing the residents to prevent injury to the resident & other residents; reports any apparent medical problem to the R.N. promptly. Will accompany & assist residents to wherever training occurs. Assists residents in the development of independent functioning in community living. Responsible for the maintenance of a safe, clean & healthful environment; reports any problems to the immediate supervisor on duty; responsible for the proper utilization of each residents' individual adaptive devices, such as wheelchairs, footboards, adaptive spoons/forks etc. Actively participates in any in-service training regarding any aspect of the residents' treatment & training with the ability to demonstrate learned skills. Drives and/or assists residents to scheduled appointment & activities; assists residents in recreational activities. Assists professional staff & team members in describing the living area & answering questions posed by families, tour groups, visitors & prospective employees.
