• Establish a therapeutic relationship and create a detailed plan of care with the patient and family, determining the most important aspects of care collaboratively.

• Evaluate the effectiveness of plans and interventions to promote the optimum achievement of established goals.

• Initiate and maintain open communication with other team members to ensure the patient and family receive comprehensive interdisciplinary expertise and services with a coordinated plan of care.

• Communicate with patients, families, subordinate staff, and peers to ensure understanding, implementation, and revision of plans and associated interventions as appropriate.

• Ensure a skills checklist completed within one year for RN specialties is included.

• Michigan license verification needed, with documentation provided if there is a license incident.

• Full-time employees from this organization are not eligible for contract hire within six months of employment.

• Weekend work is required.
