Position Summary:

Responsible for completing a behavioral health level of care assessment and providing treatment recommendations with community mental health centers and emergency departments for individuals seeking mental health/substance use concerns. Via face to face and telebehavioral health.

Job Responsibilities:

  • Participate as part of the telehealth team by completing video intakes and teleED assessments.

  • Conduct program specific screening and evaluation, and formulate diagnosis in accordance with the Diagnostic Statistical Manual (DSM 5).

  • Perform high quality clinical assessments with excellent customer service skills. Documentation is written in a proficient and professional manner. Use sound clinical judgment and develops an individualized, comprehensive treatment plan to assist client in establishing necessary mental health services.

  • Provide training and coaching support to CIC staff who are part of the telebehavioral health team and mobile outreach staff.

  • Complete regular and ongoing clinical reviews of completed intakes.

  • Responds to requests to go on mobile outreaches within five minutes of receiving call from the contact center. Must reach the designated location of the client in crisis within sixty minutes from time of dispatch for outreach or thirty minutes for TeleED assessment from the time of original call.

  • Has the ability to act as lead (primary) clinician on scene of outreach. Also able to assess the client without a back-up MOT when the outreach site is at a police station, hospital, or other secure location.

  • When acting as primary counselor, completes the Crisis Assessment Form (CAF), informed consent, and all other necessary paperwork specific to each outreach. Ensures that the call center receives the original CAF within 48 hours of completion of outreach.

Education, Experience and Skills Required:

  • Master's Degree in Social Work, Psychology, Counseling, and/or related field

  • Licensed or provisionally licensed

  • General knowledge of the St. Louis area mental health network and the DSM-IV

  • Minimum of six months to two years experience in mental health field within the private or public sector

  • Ability to write clinical documentation, conduct mental health assessments and develop client treatment plans

  • Basic knowledge of how to use the computer; basic word processing skills

  • Strong verbal, written. and interpersonal skills

They provide counseling and emotional support services to the patient and family. These caring professionals are the “sounding board” as the patient and family members work through issues that arise. They can help with Advanced Directives, funeral arrangements, recommend other community resources and agencies to assist with legal and financial needs, and can assist families in filling out necessary paperwork. Major Job Functions: Provides assistance with applications for Supplemental Security Income, Hospice Medicaid, and Medicaid Waiver; obtains resources from community social, health, pharmaceutical and welfare agencies, and provides information regarding legal forms and options. * Participates in the development of an individual care plan/discharge plan with other members of the interdisciplinary care team; communicates with other team members. * Reviews and updates the patient plan of care as often as necessary. * Ensures that physician orders are present for all social services related functions. * Participates in the quality improvement reviews and evaluations of social services. * Determines psychosocial assessment of patient needs (social, psychological, financial, cultural and family situations) for patients/families * Completes spiritual assessment on patients/families, as necessary and requests spiritual support of patients/families, as needed. * Provides range of ongoing psychosocial services to patient/family unit throughout involvement, including referrals to community agencies, bereavement care, counseling and other supportive services, as indicated. * Provides consultation to members of staff, patients/families, community agencies, and other persons or groups seeking guidance in an effort to solve the problems of patients/families, as permitted by confidentiality rules & regulations. * Provides appropriate intervention for high-risk patients/families/survivors that are experiencing suicidal ideation. * Develops and maintains close working relationship with long-term care facility staff and communicates with hospitals, doctors, and other health care providers regarding existing and potential billing problems and coordinates efforts to resolve them. * Is available for on-call. Requirements: * Applicant must have a Masters Degree in Social Work from an accredited College on Social Work Education school of social work , and meet the requirements of the state regulations. Current state license required. (State of Ohio only LSW required). * Social work experience in a healthcare setting. * One (1) year’s experience in hospice, hospital or mental health setting is preferred. * Knowledge of and ability to access community resources. * Ability to work effectively as an active member of the interdisciplinary team. Develops strong working relationships with each team member. * Ability to actively work to obtain legal/funeral home information and update team. * Able to establish effective therapeutic relationships with patients and families. * Ability to work with facility staff to coordinate facility care team meeting and oversee billing. * Must be able to maintain patient confidentiality at all times and be able to abide by HIPAA rules and regulations * Must be proficient with computers, and hand held technical devices such as Smart Phones o The ability to work in a constant state of alertness and safe manner We are proud to be an EEO employer.
