VIEW JOBS 12/4/2018 12:00:00 AM 2019-03-04T00:00 Manages and oversees an integrated medical-behavioral team that delivers direct clinical services to high risk Medicaid/Medicare enrollees across the state of Tennessee. The Clinical Manager will lead a regionally based, integrated team of nurses, nurse practitioners, social workers and behavioral therapists who complete in home clinical assessments, provide transitional care services, and identify resources to close gaps in care for dual eligible individuals who are high risk and have multiple chronic conditions. If you are located in Tennessee, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities: * Collaborate with the local Health Plan to identify, manage, and oversee an integrated set of services * Directly supervise and provide oversight and coaching of clinical and nonclinical staff * Collaborate with internal health plan leadership and external providers to support efficient, value based care * Develop and implement processes to support the clinical model of care, with a continuous improvement approach * Drive team performance to ensure key metrics and milestones are achieved * Support and implement performance improvement initiatives * Report directly to the TeamMD Market Clinical Director * Develop positive, forward thinking, and collaborative relationships with all stakeholders Required Qualifications: * MSN or MSW * Registered Nurse (RN), Advanced Practice Registered Nurse (APRN), Nurse Practitioner (NP), or LMSW - current license in the state of TN * 3 years' experience in a clinical or healthcare setting * Management and/or supervisory experience * 2 years' experience working with and documenting in EMR systems * Advanced of Microsoft products (Word, Excel, Outlook, PowerPoint) Preferred Qualifications: * 2 years' experience in a healthcare leadership role * 2 years' experience managing remote clinical personnel * Experience working with the Medicaid population * Long Term Care experience * Experience working with and documenting in eClinicalWorks EMR * Ability to drive excellence and build a culture of accountability Careers at TeamMD:.
