Responsibilities:

  • Determine the acceptability of specimens for testing according to established criteria

  • Perform routine and complex technical procedures and functions according to departmental Standard Operating Procedures.

  • Monitor, operate and troubleshoot instrumentation

  • Demonstrate the ability to make technical decisions regarding testing and problem solving.

  • Prepare, test and evaluate new reagents or controls.

  • Deliver quality patient care while maintaining efficiency.

  • Perform preventive maintenance and quality control with appropriate documentation and communication.

  • Assist with processing of specimens when needed.

  • Maintain a safe environment/wear appropriate personal protective equipment as designated by lab area

Requirements

  • BA/BS degree in Biology, Chemistry, Medical Technology, Clinical Laboratory Science, or related scientific field

  • 1 year of clinical lab experience required (or preferred at Tech Trainee level)

  • ASCP or AMT certification preferred

  • Ability to work independently within a team and strong attention to detail

  • Strong organization skills

  • Must be flexible with time and adaptable to change.

  • Must be able to pass a standardized color vision screen

  • Ability to work overtime as business needs

Summary The Integrated Behavioral Health Leader / Psychiatric Nurse Practitioner is an in-clinic role responsible for managing the Integrated Behavioral team while also functioning as a practicing provider particularly with complex patients Responsibilities and Essential Functions * Collaborate with the medical team to identify patients in need of support on health behaviors as well as those patients with more complex needs requiring behaviorally focused coordination of care * Focus on patients with complex diagnoses and poly-pharmacy needs * Coordinate with patient's medical and behavioral providers in order to ensure a holistic care plan * Screen and assess patients for common mental health and substance use disorders leveraging evidence based tools * Interact with a Patient Registry to identify, monitor, and treat high needs patients * Provide clinician and patient education about common mental health and substance use disorders, as well as available evidenced based treatment * Coordinate care, for identified patients with co-morbidities, after an acute event including emergency department presentation or inpatient admission in collaboration with the provider and population health team * Lead and participate in clinical team meetings as assigned * Document patient progress in the Electronic Medical Record Duties * Lead the Integrated Behavioral Team, including direct reports, with a goal of building a multidisciplinary team * Actively consult with the care team, and occasionally directly with patients, when there is a decline in healthy behaviors adversely impacting health condition; particularly those with complexities based on medical and behavioral co-morbidities * Prescribe medications, particularly for those patients with poly-pharmacy complexity, following the clinic's policy, formulary, and relevant regulations of the State * Collaborate with the medical team to identify patients in need of support on health behaviors as well as those patients with more complex needs requiring behaviorally focused coordination of care * Conduct a Psychiatric Case Review, monthly, with members of the Integrated Behavioral Team to provide psychiatric input on particularly complex patients * Provide consultation to the clinic's nursing and medical staff for improvement in their work performance, which enhances overall quality service delivery of the clinic * Work independently with other members of the medical team, with entitlements to specific clinical privileges approved by the clinic's Medical Executive Board * Perform patients histories and physicals, risk factor assessment, ordering of therapeutic and diagnostic tests; and attend to consultations and appointments * Screen and assess patients for common mental health and substance use disorders leveraging evidence based tools particularly when support is needed from member of the Integrated Behavioral team * Formulate, execute, and review health care education delivered to clinicians, patients, and care givers for diseases prevention; provide patient education about common mental health and substance use disorders, as well as available evidence based treatments. * Ensure smooth communication with other health professionals for effective delivering of clinic service to patients * Consult with physicians on cases that are beyond the scope of qualification of PMHNP * Ability to work in Electronic Medical Record including a longitudinal plan of care Complexity and Scope * Ability to work in a care setting as well as coordinate patients at other CMG sites * Manage Integrated Behavioral Clinicians across multiple CMG sites Physical Demands/Environmental Factors * In-clinic opportunity Minimum Qualifications * Current, unrestricted license(s) issued by the Arizona Board of Behavioral Health Examiners * Spanish speaking, or willingness to learn, preferred * Experience working in a medical setting preferred * Experience working with patients that have poly-pharmacy needs * Experience with population management This position is based in Phoenix AZ.
