Position Summary # The Ryan White Health Management Technician (HMT) position provides the critical function of coordinating the Ryan White patient care as required by the grant goals and patient individual plan of care. The HMT serves as a liaison between the Ryan White program and the facility and practice while the patient is in care, coordinating case management activities and acting as referral coordinator. The Health Management Technician reports to the Practice Manager.# The Ryan White HMT assesses patients that have been referred for care using grant program approved admission criteria. The HMT assesses prospective patient needs, works closely with referring agencies and physicians, and coordinates referrals for services, and acts as liaison between program and other points of care. Must be able to provide and understand the specific communication, developmental and treatment needs of the patients, families and visitors ranging in age from early adult to late adult. Provide ongoing communication to the care team as key member. # Minimum Requirements Education

/A # # # # Core Job Responsibilities # Primary responsibilities for program referrals, both internal and external; provide in hospital and EC patient outreach as back-up to the Outreach Worker Track outside consultation appointments and ensure patients are able to be compliant with their plan of care. The HMT will also assist in needed administrative duties, including the quarterly newsletter and organizing WAD, along with other fundraisers such as canned goods drive etc.
. Manual dexterity, good eye/hand coordination and acceptable vision needed in daily work. Must have strong knowledge of organizational dynamics and have excellent communication/coaching skills. Must be able to establish and maintain effective working relationships with patients, employees, the public, and other agencies. Must be able to respond calmly and efficiently in crisis situations. Effective verbal and written communication skills based on the command of the English language. Must be able to perform the following functions:# standing for long hours, walking, stooping, bending, sitting, pushing, pulling, and squatting. Must be able to lift 30+ lbs in order to lift/move patients, equipment, and/or supplies utilized in the work place.
