LifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando Counties.TEAM LEAD - CAT 090JOB SUMMARY: The CAT Team Lead position will supervise the Community Action Team (CAT) serving youth aged 11 to 21 and their families. The Community Action Team coordinates treatment, rehabilitation and support to youth and their family, completes timely and appropriate documentation of services and must successfully function in a fast-paced, team-driven work environment.ESSENTIAL FUNCTIONS and RESPONSIBILITIES:* The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance. * The ability to work collaboratively with others and to accept constructive feedback from supervisors and managers. * Willingness to abide by the company's published rules and regulations. * Provide management for an assigned group of youth and families including coordinating and monitoring the activities of the treatment team. * Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the other team members. * Provide individual, family and/or group therapy ensuring immediate changes are made in the treatment plans as individuals' needs change. * Educate and support individuals' families, and advocate for individuals' rights and preferences. * Conduct comprehensive assessment of psychiatric history, mental status, developmental history, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships. * Act as liaison and consult with community agencies and families to maintain coordination in the treatment process. * Document progress to maintain a permanent record of activities according to established methods and procedures. * Take the lead role or participate in providing treatment services related to SA/MH. * Provide on-going assessment of mental illness and substance use disorder, if present, and behavioral issues and the response to treatment. * Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put individuals at risk. * Provide education to individuals identifying symptoms related to their diagnosis. * May participate in setting treatment goals and plans with individuals and family to reduce or abstain from substance use and to learn replacement behaviors. * Assist the Program with monitoring, guidance, and training of Program interns. * Become certified in the High-Fidelity Wraparound Process within 3 months of employment and attend all Wraparound implementation or learning community meetings as offered or required.KNOWLEDGE AND JOB SKILLS: Must have a clear understanding of the characteristics and problems for individuals with a mental health diagnosis(es) or co-occurring mental health and/or substance abuse diagnoses. Must demonstrate excellent verbal and written communication skills. Displays excellent telephone and customer service skills. Must be able to function effectively within a team; however, must also be able to function with a high level of autonomy. Must work a flexible schedule that may include nights, weekends, and holidays.JUDGMENT/DECISION MAKING: Utilizes established corporate policies and procedures in making decisions. Graduation from an accredited school with a Master's degree in psychology, social work, psychiatric rehabilitation or a related human services discipline required. Must be eligible to be a registered intern seeking licensure with the State of Florida to be considered for the position and must apply to be a registered intern within 30 days of acceptance of the position. * Experience:*

Two (2) years of experience working with children/adolescents with behavioral health needs preferred. Two years of supervisory experience preferred. * Driving Requirements:o Must be Over the age of 21 required.o Minimum of 5-years driving experience required.o Possess a Valid FL Driver's license (within 30 days of hire if currently do not possess)o Possess a continuously valid US Driver's license for the past three years, from date of issue required.o Safe Driving record required.TRAINING & DEVELOPMENT: Completes all requirements according to Individual Training Plan within the first six months of hire and annually.
