1.Coordinates the provision of ancillary services.
A. Order laboratory, x-ray, medical supplies and equipment for patients.
B. Accurately enters all ancillary services information into the Suncoast system for tracking and follow up purposes..
C. Fax lab results to patient's primary physician(s) and relay information to Team Manager and RN for follow-up.
D. Contact patient/family via telephone to verify equipment and/or supply delivery and to introduce self and convey availability to assist patient/family.
E. Arrange for patient transportation as required.
F. Utilize Comment and Alert sections of Suncoast Communicator to notify staff of issues/changes.
2.Processes new patient admissions; existing patient discharges/
transfers; and deaths.
A. Prepare appropriate paperwork to facilitate admission/
discharge/transfer (including transfer to hospital).
B. Fax appropriate forms as necessary.
C. Broadcast message to staff regarding patient change of status.
D. Complete change of status form in Suncoast.
E. Admit family members to Bereavement program upon patient's death.
F. Call physician's office to notify of patient's death, fax letter to physician's office, and document comment in Suncoast record under MD notification comment.
3.Coordinates paperwork for team meeting.
A. Prepare team meeting agenda. Maintain binder with agenda/minutes/sign in sheets.
B. Act as the scribe at the Team Meeting capturing any patient/family changes.
C. Assist Team Manger in facilitating focused patient/family discussions.
D. Print medication sheets for distribution to RNs at team meeting.
4.Assist Team Manager and interdisciplinary team members with miscellaneous assignments.
A. Screen telephone calls for assigned Team Manager/team.
B. Run weekly forms audit report; send out Communicator to staff for follow-up; submit to Team Manager for approval.
C. Request information from physician offices (i.e., old progress reports, test results).
D. Generate daily Emergency Disaster List for Team Manager.

High School Diploma or equivalent, required. Associate's Degree, preferred.
Minimum two (2) years' office experience in a health care setting, highly preferred.
Basic secretarial skills and computer literacy - Word and Excel, required.
Knowledge, ability, and understanding of the dying and death process in relationship to the patient and family, required.
