Exciting Opportunity: Team-Based Care Planning and Operations Coordinator

This is a full-time position, working a hybrid schedule. We are located in NE Portland, near Lloyd Center. The schedule is typically Monday through Friday, daytime business hours.

Job Description: Are you ready to make a difference in healthcare coordination? As the Planning and Operations Administrator, you'll play a crucial role in monitoring and coordinating the advancement of our projects and initiatives. You'll work hand-in-hand with leadership to ensure seamless execution, promote integrated care, and foster a culture of inclusion and acceptance within our organization.

Essential Duties:

  • Monitor and support assigned projects in partnership with leadership.

  • Act as the central point of contact for internal communications related to projects.

  • Identify and coordinate training needs for project roles.

  • Collaborate with the Project Management Office to provide project management tasks as needed.

  • Develop communication plans to keep internal and external stakeholders informed and aligned.

  • Organize team-building activities, retreats, and trainings to enhance collaboration and cohesion.

  • Propose and implement process improvements to drive operational efficiency.

  • Perform other duties as assigned.

Regulatory & Compliance:

  • Ensure compliance with laws, regulations, and company policies.

  • Assist in crisis situations as appropriate, following established protocols.

  • Maintain accurate documentation in compliance with agency policies and regulations.

  • Participate in annual employee training and company meetings.

Qualifications:

  • Bachelor's degree in business administration, social science, health administration, or related field required.

  • Minimum five years of experience in program, project, or operations administration.

  • Experience leading large, complex projects and facilitating multi-stakeholder collaborations preferred.

  • Clinical or administrative experience in a healthcare setting desired.

  • Effective communication, interpersonal, and customer service skills.

  • Proficiency in Microsoft Office suite.

  • Ability to travel to multiple locations throughout the Portland Metro area.

  • Basic Life Support (BLS) certification or willingness to obtain within 60 days of hire.
