Job Summary:
Allina Health's Senior Health department is seeking a Physician with Geriatrics or PM&R experience to join our team working in the Transitional Care Unit setting. This role will allow you to work autonomously within your practice and have access to a larger team for support.
Key Position Details:
Call: Voluntary rotation shared among full Senior Health team
Average patient load: 10 per day
You would work at two primary TCU locations, depending on your geographic location. Typically you would not travel between sites on the same day.
You will see patients coming from any inpatient settings, and typically they will be in Transitional Care for 7 to 21 days
Job Description:
Principle Responsibilities
Direct Patient Care.
Reviews patient's medical history while collecting, prioritizing, and synthesizing comprehensive data pertinent to the patient's health or situation.
Evaluates, interprets, and coordinates information to make decisions and recommendations regarding patient care.
Performs comprehensive Geriatric assessments.
In transitional care and assisted living settings, manages patient post-acute care needs, ongoing primary care, care coordination and advanced care planning.
Maintains visit schedule and on-site presence requirements as outlined in program.
Provides education, consultation or intervention to patients (treatment plan) and their families.
Works collaboratively with patients to improve their health and well-being.
Practices in a thoughtful manner including consideration for cultural diversity, individual autonomy and legal responsibilities.
Charts in electronic format in accordance with licensure and scope.
Actively participates in multidisciplinary team approach to case management.
May provide care in an ambulatory clinic, inpatient or outpatient hospital setting.
May participate in on-call rotation.
Quality.
Utilizes quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes, or work flows.
May participate in peer review and quality assurance.
Other duties as assigned.
Job Requirements
Doctoral/Ph.D. level advanced degree in Doctor of Medicine, Doctor of Osteopathic Medicine or foreign equivalent from an approved school of medicine required and
Complete an Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA)accredited Residency training program in Family Medicine or Physical Medicine and Rehabilitation. Geriatric fellowship/certification strongly preferred, but not required
Licensed Physician
ACLS-BLS Tier 3 - Multisource required within 180 Days and
Board Certification or Board Eligibility in Family Medicine from the American Board of Family Medicine, or Internal Medicine from the American Board of Internal Medicine, or foreign equivalency upon hire required and
Drug Enforcement Agency Certificate upon hire required and
Prescription Monitoring Program upon hire required and
National Provider Identifier upon hire required
