System Manager of Care Management

The System Manager will assist the System Director of Utilization and Care Management in optimizing performance efficiencies within the department in alignment with system and industry standards of care

This System Manager will provide supervision of daily operations and staff development for the department's case management and care coordination teams. The System Manager will ensure appropriate staffing and assignments across units and facilities to support patient progression and well as actively orient and plan for the onboarding of new employees.

The System Manager will develop and maintain appropriate and up-to-date competencies and educational assignments across the department.

The System Manager will seek out and maintain relevant knowledge of the CMS Conditions of Participation for Discharge Planning and Utilization Review.

.Roles and Responsibilities:

  • Assist the department director providing overall direction and guidance for any clinical resource management programs and activities.

  • Supports team lead(s) in the development and improvement of department programs (UM/CDI/SS/CM).

  • Development/implementation/oversight of current employees and the on boarding for new employees.

  • Assist in the development and routine updating of existing policy/procures related to Clinical Resource Management.

  • Assist and support in all human resources functions inclusive of time, attendance, and PTO requests.

  • Supports department teams in managing patient and caregiver expectations related to discharge planning, health benefits coverage, referrals to community resources, and timely follow-up care.

  • Assumes managerial duties for performance evaluations and maintains systems to provide feedback and evaluation for team members as needed throughout the year.

  • Facilitates special projects, including those assigned by the Director, in alignment with department goals and initiatives.

Required Skills and Experience:

  • Bachelor's Degree in Nursing or Master's Degree in Social Work.

  • Current unrestricted NY state RN or LMSW licensure.

  • Minimum 3 years' experience in an acute care hospital setting.

  • Knowledge of community-based organizations.

  • Strong communication, organizational, critical thinking, and problem-solving skills.

  • Certification in Case Management or Utilization Management, or active work towards obtaining such.

Preferred Skills and Experience:

  • Master's degree in Nursing, Social work, or Healthcare oriented degree preferred.

  • Experience with multiple EMR systems, Microsoft Office, and Kronos Time management.

Physical Requirements:

  • Physical -Ability to stand, sit or ambulate for long periods. Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base.

Location and Travel Requirements:

  • Onsite at Cayuga Medical Center
  • 20% Travel can be expected in this role to partner facilities

Compensation Disclosure:

  • $42.04 to $49.25 per hour
