Responsibilities:

The Service Coordination Supervisor is responsible for overseeing the operational functions of the LTSS Service Coordination team in accordance with the AmeriHealth Caritas Pennsylvania Community Health Choices model of care.


Key Accountabilities and Responsibilities:
  • Oversees the day to day operations of the LTSS department, including the development of Person-Centered Service Plans, authorization and approval of services and the coordination of benefits for all LTSS Participants.

  • Serves as a subject matter expert for LTSS training for internal care teams and external audiences.

  • Supports coordination of LTSS with other clinical and social services across the continuum of care striving to enhance the quality of Participant care and services.

  • Maintains operational processes, policies and procedures to support LTSS care delivery

  • Ensure quality and performance metrics are consistently met regarding LTSS, as specified in the agreement

Education/ Experience:

  • Pennsylvania licensed RN, social worker or mental health professional required. (added 11-2018): On an exception basis, if reviewed and approved by the Office of Long Term Services and Support, a combination of other degrees will suffice such as MPH or other professional health care licensure. A licensure as PA RN, social worker or mental health professional must be attained within first year of employment.
  • 1 to 3 years leadership and/or supervisory experience preferred.
  • 3 years working in social service or heatlh care related field.

Other Skills:

  • Experience working with people with disabilities or seniors in need of LTSS.

  • Knowledge of the home and community-based service system and how to access and arrange for services.

  • Experience conducting LTSS needs assessments and monitoring LTSS delivery.

  • Ability to provide informed advocacy.

  • Ability to interact with health care professionals in a professional manner.

Responsibilities * Provides a full range of front desk operations in a busy primary care practice (e.g. registration, scheduling, filing, etc.). * Registers patients in timely and efficient manner. * Utilizes insurance verification systems (e.g. Navinet, Passport, Health Partners portal, etc.) to ensure insurance eligibility at the time-of-service. * Verifies patient demographics at each office visits. * Ensures that the proper areas are accessed in the Electronic Health Record (EHR) and Electronic Patient Management (EPM) system at the time-of-service. * Ensures that guardianship is verified, obtains consent to treat minor if parent did not present for appointment(s). Also obtains identification, adoption/foster care documentation as needed. * Collects co-pays (when applicable) and posts payments to patient encounters in EPM. * Represent a positive and professional image of the practice to the patients, families, community and institution through personal and telephone contact. * Schedules/confirms patient appointments via scheduling/registration system. * Records, communicates and delivers messages ensuring accuracy and timeliness. * Provides clerical functions, as needed, for the practice. * Reconcile daily fee slips as needed. * Performs charge entry as needed. * Receives, greets, directs and/or escorts patients, family members, visitors, physicians and hospital personnel as needed in a friendly and courteous manner. * Provide customer service to patients, families, physician and colleagues as needed. * Answer multi-line telephone Qualifications Bilingual/Spanish preferred Training & Education: High School graduate or equivalent preferred. Some college credits preferred. Experience: Minimum of two (2) years' experience with computer registration/scheduling system.
