The Supports Coordinator/Assistant is a key associate at Hope Network whose primary responsibilities include:

The overall purpose of the Supports Coordinator Specialist position is to work in collaboration with assigned Supports Coordination personnel to assure the timeliness and thoroughness of required documents in both traditional and non-traditional (self determination) services that are considered to be needed, but are not billable services. This additional support will further ensure the needs of the individuals served are met and are in compliance with funding agency standards and outcomes.

Essential Functions and

Responsibilities:
1. Regular and predictable attendance is an essential requirement of this position. 2.

Ability to coordinate multiple priorities with consumers, timelines and schedules.3. Provides support to assigned SC in service planning, one site referrals, program referrals and follow-up of timeliness, residential referrals, discharges and termination paperwork that also includes crisis planning and support (Gilead packets).4. Provides timely responses to the guardian and/or natural supports inquiries and/or concerns.

Maintains regular communication with other service providers as needed and required to maintain open communications.5. Responds, investigates and provides the necessary follow up when informal complaints has been received or filed on-behalf of persons served.6. Provides administrative supports to assure all required documentation is completed in a time manner and in accordance to licensing, Medicare, Medicaid, funder and agency standards including:
  • Person Centered Planning DocumentsOrganizing the IPOS packet(s). Contacting all the necessary individuals to set up the IPOS meeting, obtain signatures and following up to assure the IPOS has been implemented. Scanning all the necessary and required documents into the electronic medical record and mailing IPOS documents.Entering demographics into the electronic medical record as well as systems that have been identified by funders.Organizes data and the completion of periodic progress reports.Daily Service Activity Logs (SALS)Case notes

Maintains benefits; corresponding with persons served and the guardians regarding benefit status and/or potential issues and concerns; collecting all the necessary are required documentation needed to maintain benefit eligibility (DHS and or SSA)Coordinates meetings as needed and required with the guardian or persons served.
7.Works in collaboration with the responsible SC to provide on-going monitoring' making recommendations to the IPOS so that goals are achieved and persons served are moved to lesser restrictive environments8. Runs periodic reports and reviews data to determine productivity, billable services, and compliance with network policy C- 006.N (Establishing and Maintaining Case Records).9. Completes chart audits on a regular basis to assure service specs are being met and quality remains high.10. Reviews UR packets on a regular basis.11. Coordinates departmental and individual trainings as needed and required to maintain compliance with funder and agency expectations12. Implements and follows up on the 30-60-90 day training guide to assure all information is accurate.13. Regularly updates departmental forms and procedures to assure shared documents drives contain the most current version of forms.14. Assists with the coordination of services in the event that consumers who are referred to crisis will receive the necessary supports and/or treatment that meets the needs of the consumer.15. Ensures that persons served are informed about the opportunity for self-directed services and provides support in the pursuit and maintaining the self-directed arrangements.16. Adheres to the requirements of the Health Insurance Portability and Accountability Act of 1996 and the standards issued under the US Department of Health and Human Services and all Hope Network affiliates17. Attends departmental staff meetings as needed and required18. Licensed social workers will participate and completed the required number of CEU credits19. Participate in training that promotes sensitivity to cultural and socioeconomic diversity.

Requirements

Educational / Talent Requirements:
1. Bachelor's Degree from an accredited University.2.

Ability work independently and as part of a team. 3. Ability to prioritize and plan monthly schedules4. Ability to coordinate multiple priorities5.

Excellent verbal and written skills.13. Computer literate

Work Experience Requirements:
1. Previous knowledge of community mental health system and/or Medicaid funded services preferred but not required, 2.Experience working with the DD or MI population, 1-2 years preferred.3. Computer Literate on Microsoft Office Suite.
