Program Details: Fresh Start housing and supportive service programs for adult men and women were developed to work with individuals with a history of homelessness, chemical dependency, and/or varying degrees of behavioral health issues. Fresh Start is currently operating 265 housing beds and supportive services in collaboration with the Philadelphia and Coatesville VA Medical Centers and the Philadelphia Office of Addiction Services (OAS). In addition, Fresh Start provides transitional and permanent "self-pay" housing options. In collaboration with the Pennsylvania Department of Military and Veterans Affairs, Fresh Start also offers veterans' mobile care coordination, outreach, and advocacy services.

This is a direct care position working on the men's floors of a 45 bed residential and supportive services program for veterans located in Philadelphia PA.

  • Is the lead contact for the program on his/her respective shift, and supervises facilities and support staff on their shift.

  • Ensures all program activities and schedules are on time, and the program is operating at a high standard in accordance with local procedures, Merakey policy, and VA contractual obligations.

  • Ensures a safe, harassment free atmosphere is maintained.

  • Performs building checks for cleanliness and safety (this includes all rooms).

  • Reports all program incidents, emergencies, or issues directly to the program director.

  • Monitors residents while they take their prescribed medication, report all medication monitoring issues to immediate supervisor and following Merakey incident report policy 702.00.

  • Ensures nutritious meals are being served during their shift.

  • Assists case managers and facility attendants with case management tasks (i.e. assist veterans with paperwork, provide veterans with linkage to various resources, assist veterans with online applications and resources, etc).

  • Ensures that scheduled social, educational, and therapeutic activities are provided in accordance with program director/case management supervisor's direction.

  • Coordinates veteran transportation needs with driver on his/her shift.

  • May perform additional duties as required or assigned by supervisor or program director.

Schedule: Sunday through Thursday 11:00 PM to 7:00 AM

  • Bachelors' degree in social work or human services field, or high school diploma with 5 years minimum experience in a direct care position.

  • Three years of management experience.

  • Prior experience in working with veterans is preferred.

  • Proficient in Microsoft software (i.e. Word, Excel, Power point, and Outlook.
