Job Summary:

The position participates in maintaining indexes and logs and classifying chart information, reviewing documents for accuracy.The position is responsible for managing the systems, procedures and policies governing clinical documentation and accurate coding.

Responsibilities:

  • Track/ Reconcile missing or outstanding billing encounter forms

  • Reconciliation of clinical documentation and billing encounter forms

  • Send e-mails requesting addendums and e-signatures, check for them before billing

  • After completing a date of service reconcile all the names in the schedule

  • Give all encounter forms to registrar once billing is complete

  • Manage Bariatric Consults

  • Organization and reporting of quality data

  • Track discharges and admissions to the clinic

  • Label and file all intake charts

  • Scan charts into Electronic medical records

  • Accesses protected health information (PHI) in accordance with departmental assignments and guidelines and maintains compliance with HIPAA, local, state, federal, and other agencies rules and regulations.

  • Reviews and checks records to insure legal and hospital accreditation requirements have been met.

  • Other duties as assigned by Employer.

Qualifications/Requirements:

Experience:

  • Knowledge of medical terminology and modern medical records procedures

  • Ability to effectively use computer applications such as spreadsheets, word processing, calendar, e-mail and database software in performing work assignments.

  • Ability to communicate effectively, verbally and in writing and to understand and administer rules, policies and procedures, to follow oral and written instructions and to read, understand and apply rules.

Education:

MINIMUM ACCEPTABLE TRAINING AND EXPERIENCE: Possession of a high school or

equivalency diploma and either: (a) one year of clerical experience in a health care setting; or (b) satisfactory completion of 60 credits* from a college or university, business school, or (c) a satisfactory equivalent combination of training and experience as defined by the limits of (a) and (b).
