The Support Coordinator - Memory Disorders Center ensures timely access and seamless coordination of services throughout the continuum of care for patients with a memory disorder and/or dementia. Follows up on treatment plans to coordinate care and assists in implementing recommendations.
Serves as a liaison to patients, community physicians and services to facilitate and enhance the quality of care of the patient. Provides information, education and shares knowledge regarding internal and external support services and resources. Facilitates scheduling, coordination follow up care, and tracking to ensure completion of recommended treatment plan.
Serves as an integral part of the interdisciplinary team promoting optimal patient care. Provides administrative support to the department; including maintaining records, scheduling and coordinating meetings, inventory and ordering of supplies, greeting and directing patients, visitors and team members, word processing and computer program use, answering and directing calls, recording minutes at assigned meetings, time and attendance.
