To achieve this unwavering support, Familylinks provides a wide range of family-centric services encompassing Behavioral Health, Youth & Family Services, Service Coordination, and Senior Services. The role of a Support Coordinator (SC) encompasses coordinating support services to persons with intellectual disabilities and/or Autism and their families/caregivers. Major functions involve locating, coordinating, and monitoring the services received by individuals enrolled in the program as well as assisting in caseload coverage and critical revisions to support plans. Casual Support Coordinators may transition to a full-time position, if needed.

Requirements

A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years' professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.
A valid driver's license along with access to a vehicle.
Proficiency with MS Office, computer/laptop, tablet and cell phone required.
