Support Coordinators provide support and resource identification functions to customers recovering frompsychiatric illnesses under clinical supervision and assists with treatment; may provide substance abuse services,education, support and consultation to families; provides crisis intervention as appropriateand serves as integral member of the crisis counseling, stabilization andtherapy team.

Assists clients in gaining access to needed financialand insurance benefits, employment, medical, social, education, and otherservices. Provides information andreferrals to assist clients' with needs such as medical, dental, occupational,residential, social and any other identified barriers to stabilization. Workswith the client and the client's natural support system to meet the needs ofclients.

Completesthe community support assessment as well as other assessment forms such as, butnot limited to the Bio-Psychosocial Preliminary Assessment, MedicalQuestionnaire, and the Medication / Verification form. Based on the assessment, develops inconjunction with the client / significant other(s), a service plan to addressthe needs of the client.

MeetsService Productivity standard of at least 75% (or 6 out of 8 work hours) forthe provision of Case Management services in accordance with applicableclinical/quality criteria which are supported by appropriate clinical recorddocumentation which meet Client Record Handbook specifications, SAL documentationrequirements, and Agency business/clinical ethical standards.

Providescoordination of care between service providers, Crisis Stabilization Unit(CSU), and other necessary services. Makes appropriate referrals to community resources to assist clients tomet their needs and goals.

Documentsall services provided in the client record in accordance with the Client RecordHandbook Guidelines. Progress notes mustbe clearly reflected how the support coordinator's efforts are linked to theservices and the client's progress or lack of progress on each client'sgoals.

Maintains at least 85% compliancewith progress note requirements on Clinical Reviews conducted during ratingperiod. Program case files must beconsistent with Center standards, to ensure compliance with Agency policy,F&F rules and JCAHO standards.

Identifyhigh risk clients and complete suicide risk assessments for all clients duringthe intake and/or at anytime during treatment a client are noted to havesuicide ideation, plans, or an attempt. Reviews risk assessments with the Supervisor and /or MD/ARNP uponcompletion.

Includes suicide risk,ideations, plans, or recent attempt to determine appropriate intervention toensure the safety of the clients. Assists the client in completing the Personal Safety Plan. Maintains at least 85% compliance withassessment requirements on Clinical Reviews conducted during rating period.

Trainedin SSI/SSDI/ Outreach, Access, and Recovery (SOAR) and assist eligible clientsgaining SSI/SSDI.

Attendsand participates in staff/unit meetings as required.

Education and Experience:

  • Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field.

  • Or other Bachelor's degree from an accredited university or college and one (1) year full time experience working with adults experiencing serious mental illness.

  • Ability to manage a large caseload in the provision of focused, short term services.

Shift: Monday

  • Friday, 8:00 a.m.

  • 5:00 p.m.

Rate of Pay: $12.50/hr

To be considered for employment, you will be required to undergo a criminal background check, employment references check, and drug screening. In addition you must have a current and valid driver's license with no more than 6 points. A 3 year driving record will also be required upon hire.

Hours: Monday -Friday, 40 hours General Summary: Works closely with Retail Support Group Manager to ensure all areas of responsibilities are accurate and completed timely within the department. This includes all Teller, Personal Banker, and Retail Support procedures and objectives. All associate at Capital City Bank have access to confidential client information, and must practice discretion at all times. Principal Duties and Responsibilities: * Has a clear understanding of the policies and procedures for processing teller transactions as well as cross-trained for other roles within the department regarding documentation review and personal banker reporting. Provides general operational support to Retail Office Delivery as needed. * Evaluates and analyzes effectiveness of teller and retail support operational policies and procedures. * Maintains and revises the Teller Manual to reflect current policies and procedures and communicates additions/deletions/revisions to affected associates when needed. * Prepares various weekly and monthly reports for distribution to Retail Office Delivery and services as back up for other various reports required for distribution monthly. * Serves as support in assessing new technologies impacting teller operations. * Prepares content and facilitates lead teller meetings. Assists in delivery of teller training. * Responsible for the Performance Standards responsibilities including over and short general ledger entries, cash items and charge-backs for possible cash losses and/or disciplinary action, communicating to management by way of written documentation as needed. * Works with vendors for office cash needs and coordinates deliveries as needed. * Assist in maintaining official audit records for ATM, Teller Drawers, Vault, and TCR and creates reporting as needed. Associates will consistently follow policies and procedures as established by Capital City Bank, with department managers setting the standard. Knowledge, Skills and Abilities (KSA) Required: The following KSAs are usually acquired through college education with specialized training course(s) in business or banking and five to seven years substantive operational experience: * Strong written and oral communication skills * Excellent critical thinking and decision making skills * Proven management skills * Extensive background in bank policies and procedures, state and federal banking regulations and associate development * Proven experience in work requiring detail orientation * Proficiency in Word and Excel software Education: Associate's or Bachelor's degree in business or finance from an accredited college preferred.
