We are seeking a Home Health Supervisor of Patient Care Coordination to work in the Hancock, Michigan area. The Home Health and Hospice Supervisor of Patient Care Coordination (PCC) provides clinical and administrative supervision to the Home Health and Hospice (HHH) locations in addition to functioning as a Registered Nurse. The HHH PCC is responsible and accountable for the delivery of safe and high quality home health services and is an advocate for the patients under their care.
The Registered Nurse is a professional who assumes the responsibility and accountability for the provision of assessment, planning, implementation and evaluation of nursing care provided that incorporates evidence-based practice. A key aspect of the work of Nursing is to continually improve the quality of nursing care by collecting and analyzing data.
Knowledge of the principles and practices of nursing, ability to implement the nursing process, and skills in techniques of nursing care normally acquired through completion of an Associates or Bachelor's degree in nursing required.
Possesses an active Wisconsin and Michigan RN nursing license.
Current BLS certification or obtained within 90 days of hire.
Valid driver's license required.
Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability.
1 or more years' experience in home health care preferred
Supervisory or management experience desired.
Able to handle stress and adverse conditions calmly, pleasantly, and politely.
Hours: This position is full time; 1.0 FTE or 80 hours per pay period. Full time benefits package available.
