Supervisor, (Rn) Registered Nurse Case Management

Job Description

Supervises, instructs and guides clinical and non-clinical staff in the case management of at-risk patients. Interprets and implements the department's policies and objectives. Functions as a leader to interdisciplinary team of clinical and non-clinical staff.

Job Responsibility

• Promotes department goals by selecting, motivating, and training capable team members. Leads the activities of assigned team members by communicating and providing guidance toward achieving department objectives.
  
  Supervises the staff in the general performance of their duties and trains assigned staff, ensures performance appraisals are completed in a timely manner. Coordinates multi-disciplinary case conferences with Sr. Director, Director and/or staff to review member cases.
  
  Directs and monitors the appropriateness of scheduling and assignments. Participates-in evaluating and implementing on-going functioning, growth and development of the department. Collaborates with management to ensure development of high standards of quality care.
  
  Assists with data collection for measurement of productivity. Prepares monthly reports on statistics. Interacts with insurance managed care contracts to maintain standards of care provided to members.
  
  Participates in the development of the annual program evaluation and ongoing revision of the policy manual, department procedure manual, goals and objectives. Reviews documentation submitted by staff to ensure adherence to standards of quality, appropriateness and compliance with regulations. Performs related duties as required.

Job Qualification

• Bachelor's degree required, or equivalent combination of education and related experience.

• Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

• 4-6 years of relevant experience, and 0-2 years of leadership/management experience required.
