Responsibilities

Position Summary:

Supervises a unit of service that contributes collectively to the provision of high quality patient care. Monitors and supports the desired outcomes as established for their specific area of responsibility.

Qualifications

Minimum Qualifications:

  • RN or PT or OT or SLP or MSW, or LCSW license

  • Bachelor's degree in a Health or Business related field

  • Minimum of two (2) years of management experience (within the last 5 years) in a home health agency, primary care clinic or health facility; sufficient background knowledge and expertise in clinical decision making for the patient population assigned

Preferred Qualifications:

  • Master degree in health or business related field

  • Supervisory experience preferred

Responsibilities The Home Health Registered Nurse is a professional who provides and supervises care to individuals using the nursing process. Consistently provides a safe and confidential environment. Provides skilled nursing to the sick and disabled in their homes or place of residence. Performs age specific assessments and based on the assessment, and with the patient, family, and physician develops a plan of care. Works in collaboration with other team members in the Home healthcare system; insures documentation reflects care; and is responsible for providing leadership and maintaining personal professional development. Qualifications Knowledge of current nursing care techniques, principles and procedures. JCAHO, Title 22, Medicare regulations pertaining to Home Health. The Nursing process. Ability to work as a team member to meet the department goals and objectives. Treat co-workers, patients, physicians and family members with dignity and respect. Maintain patient confidentiality. Communicate effectively with team members in verbal and written form, using electronic clinical information system. Establish and maintains a cooperative working relationship with physicians, nursing staff, therapist, other hospital departments, and outside agencies, Maintain knowledge of changes governing Home Health Agencies in accreditation, State and Federal Regulation, third party payors, and patient care needs. Work a flexible schedule and be willing to work overtime if needed for the provision of quality patient care. Function autonomously. California RN license, BSN with PHN preferred.
