Position Summary

The purpose of this position is to care manage patients/residents as assigned, collaborating with and supervising other disciplines and services. Such collaboration must be in accordance with current federal, state and local standards and guidelines to ensure that the highest degree of quality is maintained at all times.

Responsibilities

ESSENTIAL:

Responsibilities:

(40%)* Responsible for the assessment, development and ongoing changes of patient/resident care plans, and quality outcomes.

(25%)* Participates in direct patient/resident care as needed.

(20%)* Ensures appropriate care plans are initiated and updated with condition change, and documentation supports care provided.

(15%)* Daily rounding with staff and patient/resident.

OTHER DUTIES AND RESPONSIBILITIES:

Develops and maintains good working rapport with inter-departmental personnel to meet the needs of each patient/resident.

Completes performance appraisals for assigned staff with collaboration of manager.

Conducts counseling/disciplinary meetings with staff with collaboration of manager.

Addresses and completes appropriate follow through on: Patient/resident/family concerns/complaints Staff concerns Variances (including investigation) Follows the Policy & Procedure, written guidelines, and standards.

Makes recommendations for development and/or revisions.

Communicates complaints, problems, and/or concerns to the DON, Chief Nursing Officer and/or Administrator.

Participates in Performance Improvement.

Prepares/maintains necessary records and reports.

Participates with manager regarding staff scheduling to meet the needs of the facility, assures staffing levels to meet patient/resident needs and meet regulatory standards.

Assists with the orientation and ongoing competencies in collaboration with manager.

Instructs other team members when appropriate, explains goals and rationale for individualized care.

Collaborates in the development, planning and presentation of education activities.

Participate in regulatory surveys.

Actively participates or leads committees as assigned or appropriate.

Qualifications

Education:

Essential:

  • Associates Degree

  • Nursing

Education equivalent experience:

Essential:

  • Bachelor's or Master's degree in nursing or related field is preferred. Minimum of two years' experience in a long term care facility or acute care, preferred.

Credentials:

Essential:

  • Registered Nurse

EXPERIENCE, TRAINING AND SKILLS:

Demonstrated leadership abilities; able to work under stressful situations; and displays use of professional verbal and written communication skills with integrity and initiative.

Read, write, speak and understand the English language.

Ability to make independent decisions when circumstances demand.

Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.

Willing to seek out new methods and principles and incorporate them into existing practice.

Physical/Mental Requirements and Working Conditions

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.

Some exposure to blood borne pathogens and other potentially infectious material. Must follow MidMichigan Health bloodborne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.

Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.

Physical Demand Level: Medium. Must be able to occasionally (0-33% of the workday) lift or carry 21-50 lbs., frequently 34-66% of the workday 20 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls and constantly (67-100% or the workday) 10 lbs.

The target patient/client population for this position are those requiring skilled clinical home care services. Activities of this position are patient/client focused and utilize the nursing process of assessment, planning, intervention, and evaluation in accordance with nursing practice standards, physician orders, and agency policies and procedures. Responsibilities ESSENTIAL: Responsibilities: (20%)* Regularly assesses patient's/client's needs and provides skilled nursing services as prescribed by the physician authorized plan of care and accepted standards of nursing practice. (20%)* Initiates plan of care for assigned patients/clients and revises and adapts as necessary. (20%)* Provides nursing care (or arranges for services through specialized staff) through ongoing visits to a group of patients/clients, sharing the patient/client case load. (15%)* Coordinates patient/client care by evaluating outcomes and providing on-going feedback to others involved in care relative to patient status and changes needed on patient care plan. Establishes a Home Health Aide care plan and supervises the Home Health Aide according to Agency guidelines. May make joint visits with other disciplines to assure consistency of care. Functions as part of multi-disciplinary team in coordinating care including planning for discharge. Acts as a preceptor to provide educational visit opportunities for students and will participate in orientation of new staff as assigned. Participates in Agency, community and/or home health organization activities as assigned appropriate.
