Responsibilities/Job Description

The Supervisor of Hospice Social Work and Bereavment supervises the activities of the Hospice Social Work and Bereavement departments. Works with Hospice Social Work & Bereavement Manager with hiring, recruitment, supervisory visits and strategic planning. Assures quality care, regulatory compliance and customer satisfaction.

Responsibilities include:

  • Works with Manager of Social Work (SW) & Bereavement to oversee day to day operations for Social Work and Bereavement.

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.

  • Actively participates in creating and implementing improvements.

  • Assesses department's human resources ensuring proper utilization of human resources and positive employee relations. Recommends performance standards, reviews performance, provides coaching, feedback and recognition on job performance on an ongoing and timely basis.

  • Drives employee selection and ensures employees are orientated to the organization, business unit and position.

  • Assists in the financial forecast and monitors forecast for monthly variances. Identifies, recommends and works with Manager on implementating changes that will improve productivity and/or financial performance.

  • Works with Manager of Social Work and Bereavement on day-to-day operation of Social Services and Bereavement to ensure the needs and expectations for Hospice services are met. Evaluates all service components.

  • Supervises and provides clinical direction to Social Work and Bereavement Staff. Monitors and supervises total patient care to ensure efficient and effective delivery of Hospice Services. Consults with interdisciplinary team to organize and direct patient care.

  • Evaluates clinical competency through direct observation, skills assessment, and performance appraisals. Oversees compliance with timely submission of all documentation. Supports and creates a culture of accountability for Conditions of Participation integrity and quality outcomes.



  • Master's degree in Social Work

  • 2 years management experience in hospice and/or homecare or equivalent combination of education and experience.

  • Knowledge of counseling therapy and practice. Specialized knowledge in the field of Hospice Services. Knowledge of home care and/or hospice state and federal standards/regulations.

  • Licensed Independent Clinical Social Worker (LICSW) or Licensed Graduate Social Worker (LGSW) by MN Board of Social Work

(ILO), is a Minnesota based organization that provides services and supports for individuals with a range of disabilities and/or mental health diagnoses. ILO is seeking individuals to work Full-time and Part-time positions in our Community Support Services (CSS) Division. The primary responsibility of the CSS positions is to provide services to individuals by consistently meeting and assessing the individual's goals and needs (per guidelines set by DHS and the agency). Individuals who are employed in the CSS Division are assigned a caseload of clients in a central location; however, a caseload could span the seven-county metro area depending on client needs. The CSS Division encompasses the following positions: Behavior Specialist (BS), Housing Access Coordinator (HAC), Independent Living Skills (ILS) Specialist, and Individualized Home Supports Staff (IHS). The role of the Behavior Specialist is to assist and respond to clients who engage in maladaptive behaviors that negatively impact independence and self-sufficiency in the community. The BS works closely with the client, client's support team, and Positive Behavior Supports team to provide direct care to clients utilizing a person-centered, positive behavior modification plan. Job duties include: observing, collecting and recording behavior data, communicating questions or concerns to the client's team and Positive Behavior Supports team, and client documentation. The BS must have at least 2 years of supervised experience working with individuals with disabilities and/or challenging behaviors and/or a degree signifying experience within the field (i.e. Psychology, Social Work, Human Services, etc.). The role of the HAC Specialist is to assist clients with obtaining and maintaining both temporary and permanent housing. More specifically, HAC Specialists aid clients with determining prospective housing options based on the client's preferences and needs. This includes but is not limited to: identifying the client's budget, researching prospective housing amenities and locations, assisting with touring, and accessing other financial resources that could assist with solidifying housing. This aid occurs at both the planning, researching, and moving points of housing acquisition. It is preferred that HAC staff have at least 1 year of experience working with individuals with disabilities and/or mental health diagnoses. The role of the Independent Living Skills (ILS) Specialist is to assist clients with increasing their independence and self-sufficiency within their community. Examples of work performed with clients include but is not limited to: finding and accessing community resources (food shelves, housing, employment, etc.), budgeting and financial management (bill pay, balancing accounts), household management (grocery shopping, meal planning/preparation, sales/coupon use, etc.), community integration/socialization, symptom management, etc. It is preferred that ILS Specialists have at least 1 year of experience working with individuals with disabilities and/or mental health diagnoses. The role of the Individualized Home Support Staff (IHS) is to provide direct supervision and training for people who live in community settings. Broad categories for support include community participation, health, safety and wellness, household management, and adaptive skills by offering guidance and instruction to the client. Persons served are typically seen multiple times weekly and may require remote support for the purposes of check-ins and consultation. It is preferred that IHS staff have at least 1 year of experience working with individuals with disabilities and/or mental health diagnoses.
