Conditions of Employment

• As a condition of employment, newly hired employees are required to be fingerprinted to provide information for a Washington State Patrol and FBI History background inquiry at a personal cost of approximately $53.00. Must be completed PRIOR to starting employment. Do you agree to this condition?

• An inquiry may be made to the Washington State Patrol, a Federal or other Law enforcement agency to verify your responses to the Background questions. A copy of any response received pursuant to such inquiry will be made available to you upon request. By answering yes to this statement, I certify under penalty of perjury under the laws of the State of Washington that the foregoing is true and correct.

• In consideration of the South Whidbey School District's review of this application, are you authorizing and releasing the South Whidbey School District and all providers of information regarding your personal, educational, vocational or employment history from any liability as a result of furnishing and receiving any information in the verification of your answers to the Background questions?

• Are you a citizen of the United States or have a visa which permits you to work in the United States and are you able to provide official documentation as proof of eligibility prior to employment?

• Do you understand that any falsification, misrepresentation or omission of facts on the application or during the interview process shall be sufficient cause for disqualification of the application or termination of employment and that this application and records become the property of the South Whidbey School District, which reserves the right to accept or reject it?
