Substance Use Coordinator

General Description:

The Substance Use Coordinator is responsible for the coordination of services for individuals diagnosed with behavioral health disorders (substance use and mental health). Services are provided in conjunction with other services such as psychiatric rehabilitation, residential rehabilitation, health home and supported employment. The Substance Use Coordinator works directly with individuals served, the rehabilitation team and outside service providers to ensure the supports are in place for individuals to overcome addictions and maintain long term sobriety. Schedule: Full time. This person reports to the Psychiatric Rehabilitation Program Director.

Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:

A Bachelor's degree with one (1) years' experience in addiction services is preferred. Related field certification is desired for this position. A good driving record is also essential. Leadership qualities and the ability to consult with others as a part of a team are necessary. Ability to respect individual choice in the recovery process and a commitment to psychiatric recovery is required. Also, this position requires excellent verbal and written communication skill proficiency with an electronic health record.

Summary of Essential Functions, Responsibilities and Duties

  • Develop initial assessment and Co-Occurring contract with incoming persons served.

  • Act as a partner in recovery to coordinate substance use services such as relapse prevention, drug and alcohol education, and recovery sustainment through group and individual settings.

  • Assist individuals with community resource linkage such as public transportation, benefits coordination, physician appointments, substance use treatment/counseling and overall development of independent living skills as needed.

  • Complete annual, bi-annual, monthly, and daily documentation in the electronic health record for caseload of individuals.

  • Assist with locating and attending self-help groups (AA/NA).

  • Administer and monitor random toxicology screens based upon individual needs as stipulated by the individual's service plan.

  • Provide off-site services in setting of the individual's choice (residence, community setting).

  • Coordinate social activities to assist with individuals' developing a sober social network and to assist with the acquisition of appropriate socialization skills.

  • Engage in crisis management and be available for consultation and support after hours and on weekends.

  • Perform other duties as requested by PRP Director.
