Responsibilities Complete, in collaboration with the training department, all Screening and Enrollment of Persons served referred to ACCS within prescribed timelines Screen, assess, and reassess people served for substance use, addiction risk, and addiction treatment needs. Develop formulations that address the level of use and potential risk, the current impact of substances, and the Person's readiness for change and support the Person's relapse prevention and long-term recovery.
Identify and evaluate Persons served risk issues and develop appropriate critical need interventions and transition plans. Ensure timely completion, with Persons served and other members of the Trans-Disciplinary Team, of the Comprehensive Assessment and other clinical assessments as needed. Participate in and collaborate on the development and implementation of Treatment Plans and Treatment Plan reviews.
Develop and facilitate clinical and support groups based on the needs and preferences of Persons served. Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with people served and family members. Coordinate with Care Coordination Entities and other providers to ensure that Persons have access to services that address addiction treatment and related issues.
Demonstrate respect for all Persons regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity. Provide direct interventions and supports to people served based on their stage of treatment or stage of change using evidence based and promising treatment practices. Provide clinical consultation to staff teams and assist the team in understanding how the use of substances and/or addictive behavior can impact a person's recovery.
Provide agency staff and Persons served with ongoing support and consultation; respond to serious events as needed. Provide modeling, supervision, and training to direct care staff, especially related to evidence-based interventions. Develop, coordinate, and deliver trainings in conjunction with the Integrated Team members, as needed to address the clinical learning needs of staff (e.g. orientation, crisis intervention, developing a clinical perspective, etc.). Develop, coordinate, and deliver trainings in conjunction with the Integrated Team as needed to address the clinical learning needs of staff (e.g. orientation, crisis intervention, effectively dealing with challenging behaviors, etc.). Develop productive affiliations and partnerships with other community-based providers to enhance the agency's service delivery network, ensuring people have appropriate access to addiction services as needed.
Assist program leaders to identify and evaluate referred Persons' risk behaviors and develop appropriate transition plans and interventions. Coordinate risk management data and develop and implement corrective action plans in conjunction with other members of the ACCS Team. Support and monitor the implementation of identified clinical approaches and evidence based practices ascribed to by the agency.
Participate in On-Call rotation and critical incident review process. Perform other duties as assigned. Qualifications
Experience working with adults with mental illness, emotional disturbance and co-occurring substance abuse disorders, with one of the following licenses: LADC I with Master's in a behavioral health field; LADC II with at least 2 years of experience working with individuals with substance abuse disorders; or CADS/CADC II with at least 2 years of experience working with individuals with substance abuse disorders and a valid driver's license required.
