SCOPE OF POSITION: The Stroke Coordinator, reporting to Vice President of Patient Care and Chief Nursing Officer, is responsible for the administrative and clinical support of the hospital#s Stroke Center.# Administer and maintain the standards of practice for the care of stroke patients and interaction with all healthcare providers, patients, families and the community regarding these standards.


Duties include:


communication and direction of clinical staff to comply with standards of practice; medical record review; data collection, analysis and reporting; regulatory and accreditation compliance; staff education and training; patient and family education; policy and procedure development implementation and maintenance; community outreach. Reports to the Administrative Director and or/Nurse Manager.


RESPONSIBILITIES:


Provide administrative support to the hospital-wide stroke program, including application and ongoing Primary Stroke Center certification. Serve as primary liaison with CT Department of Public Health for the purpose of this certification.

Establish and implement medical record review process.# Perform concurrent and retrospective review of medical records to abstract clinical data on stroke patients.

Provide timely feedback and prompting of clinicians to ensure medical record accuracy and completeness.# Maintain the #Get With the Guidelines# AHA data base. Day-to-day chart review of stroke patients and coordination of multidisciplinary services.

Meet with families of newly diagnosed stroke patients. Coordinate the development, periodic review, updating, and distribution of patient/family educational materials and monitors availability of educational materials in patient care areas. Collaborate with other stroke team members to develop annual education plan to ensure stroke center certification education requirements are met; prepare educational materials for outreach events, stroke screenings and lectures; serve as hospital liaison to AHA local chapters to promote stroke awareness within the local community; maintain records and schedules of required community education to demonstrate compliance with Stroke Center Certification.

Plan in-service education for staff as needed to address knowledge deficits and develop education plan, including annual grand rounds program for hospital staff. Act as the liaison between prevention personnel, pre-hospital personnel, EMS, rehabilitation facilities. Act as an internal and external consultant on stroke-related efforts.

Facilitate stroke prevention community outreach efforts. Participate in state-wide stroke meetings/conferences, coordinating and brainstorming with other CT hospitals. Perform all other duties as assigned.


REQUIREMENTS:


Current Connecticut RN license and minimum 5 years of current clinical experience. Working knowledge of CT DPH certification requirements and Joint Commission requirements related to the Hospital#s program for care of the stroke patient. Knowledge of pathophysiology of cerebrovascular disease. Knowledge of care delivery processes and related medical record documentation tools.
