Charlotte Hungerford Hospital is a 122-bed, general acute care community hospital located in Torrington, Connecticut, that serves as a regional health care resource for the 100,000 residents of Litchfield County and Northwest Connecticut. In conjunction with the Stroke Center Medical Director, the Stroke Coordinator is responsible for the development, implementation, coordination and evaluation of the stroke program, adhering to the Clinical practice guidelines and Joint Commission standards for quality care. In conjunction with the Stroke Center Medical Director, the Stroke Coordinator is responsible for the management, education and research related to stroke care and prevention. The coordinator provides support for the stroke program to medical and nursing staff and those hospital departments, which have direct and indirect patient care responsibilities. The stroke coordinator is responsible for problem identification, problem solving, education, performance improvement and research related to stroke care.
Provides leadership for the development and maintenance of guidelines and order sets for the continuing care of stroke patients to ensure that best practices are followed.
Offers consultative services and collaborates with the interdisciplinary staff to serve as a resource and provide direction with clinical decision making to the individuals providing care to stroke patients.
Plans and coordinates the initial and ongoing training/education of the multi-disciplinary team.
Serves as staff support for stroke team.
Tracks and reports on specific performance and outcome measures.
Schedules/coordinates stroke team rounds.
Guides, supports and executes ongoing Performance improvement projects related to improving the care and outcomes of stroke patients. Also, uses data to support ongoing initiatives.
Monitors performance indicators, quality of care issues, cost of care and guidelines for stroke care management, including patient education.
Acts as clinical liaison between physicians, clinical coordinators and administrative staff.
Plans and implements ongoing formal and informal educational programs related to care of the stroke patient for nursing and other disciplines.
Develops and delivers stroke specific education to patients and families as well as the care team involved in the care of stroke patients.
Actively participates in stroke conferences and nursing education for stroke care.
Participates in the development of care services at the community, state, regional and national levels.
Maintains all pertinent data related to stroke program activities.
Monitors all research activities for the Stroke Center.
Prepares annual report to Joint Commission.
Develops and delivers education to local EMS providers in order to maintain high levels of pre-hospital care.
Attends professional meetings and workshops which enhance professional growth. Maintains a minimum of 8 hours of stroke specific CEU's annually.
Participates in system-wide development of Ayer Neuroscience Institute stroke initiatives.
Actively participates in stroke conferences and nursing education for stroke care. Prepares educational materials for lectures, health screenings and outreach events.
Master's degree in health related field required
Three years of stroke/neurology experience required
