SUMMARY:

The Case Manager II, Street Outreach works as a member of a multi-disciplinary team providing assessment, individual planning, street outreach, intensive case management, peer services, and service brokerage. The team coordinates with community resources to provide access to address all needs, including substance use treatment, medication management, physical health treatment, mental health treatment, benefits and entitlements, appropriate and affordable housing, employment and educational opportunities, and supports for basic needs. This role will be someone experienced in outreach services that can provide guidance to level I Case Managers in LEAD.

RESPONSIBILITIES:

HOUSING DUTIES

  • Provide client housing assessments and housing focused case management to identified LEAD participants and develop patient identified goal driven plans. LEAD CM will defer to LEAD Housing CM for all ongoing housing CM needs for active participants.

  • Provide ongoing individual support and encouragement to clients, including transportation to and from community services to access housing.

  • Pursue affordable housing options for CCC participants and assist them in accessing affordable housing, including in helping to access needed emergency and ongoing funding for housing.

  • If a LEAD participants moves in some type of housing (shelter, transitional or permanent), the LEAD housing CM will create and identify ongoing LEAD housing goal to determine if LEAD or other community partner will be primary case manager for housing.

CASE MANAGEMENT

  • Engage referred individuals and collect information regarding individuals' history and current presentation through a

  • combination of interviewing, records review, and consultation (after obtaining ROI and written consent).

  • Perform risk assessments and evaluate level of care needs.

  • Develop a participant-directed, strengths-based plan with individuals using a harm reduction approach.

  • Work with community agencies and programs to broker services for referrals as identified on plan.

  • Provide outreach in the community to locate and engage participants.

  • Participate in Outreach activities necessary to offer services and engage participation in a non-coercive manner.

  • Escort individuals to meetings and appointments as necessary.

  • Participate in admission and discharge planning for any client needing hospitalization or subacute level of care.

  • Provide testimony and advocacy with courts and other agencies.

  • Collaborate with police, sheriff's department, courts and other agencies.

  • Participate in training opportunities to advance skills and knowledge in addictions and mental health.

  • Participate in staff meetings, huddles, and case conferences with team members in order to utilize interdisciplinary

  • expertise and facilitate individual service plans.

  • Participate in collaborative Operational Team meetings with partner agencies.

  • Monitor and assure that the health and safety needs of individuals are met.

  • Utilize Motivational Interviewing to identify stages of change and develop corresponding stage wise treatment

  • Maintain required records.

  • Work within scope of training and experience and seeks consultation when appropriate.

  • Adhere to all state and federal privacy regulations and to CCC policies and agreements regarding confidentiality,

  • privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

  • Perform other duties as assigned.

OTHER DUTIES IF APPLICABLE:

  • This role works in collaboration with the Milwaukie Police Department.

  • Responsible for outreach and short term case management in partnership with Milwaukie community partners.

  • Attend regular police briefings and Milwaukie outreach meetings as needed.

ABILITIES:

  • Basic knowledge of Motivational Interviewing, Stages of Change, Harm Reduction and ASAM.

  • Demonstrated knowledge of community and social service agencies, including health services (substance use, mental health, and physical health), housing, employment, benefits, and other social services.

  • Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.

  • Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.

  • Ability to set boundaries, resolve interpersonal conflict and de-escalate issues.

  • Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving.

  • Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.

  • Ability to work effectively with peers, individuals and groups.

  • Ability to work independently and as a team member and to establish priorities.

  • Ability to apply principles learned to new situations.

  • Ability to learn new skills.

  • Ability to display a non-judgmental attitude.

  • Sufficient manual dexterity and physical ability to perform assigned tasks.

  • Knowledge of de-escalation methods or ability to be trained in de-escalation methods.

  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.

  • Must be able to climb stairs several times a day.

QUALIFICATIONS:

  • HSD/GED required.

  • Education and experience appropriate for this role, as demonstrated as one of the following: QMHA, CADC, Bachelor's Degree in service related field preferred.

  • Experience working with homeless individuals and providing street outreach, or outreach based community social service or healthcare environment preferred.

  • Lived experience preferred.

  • Bi-lingual preferred.

  • Must have current CPR certification prior to start date.

  • Has the competencies necessary to:

  • Communicate effectively.

  • Understand treatment and service terminology and able to apply the concepts.

  • Assist participants in developing a self-directed plan.

  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.

  • Must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.

  • Must adhere to agency's non-discrimination policies.

  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations and treat each individual with respect and dignity.
