The Transplant Nurse Coordinator is responsible coordinating new patient referrals, evaluation, listing, and transplant care management of patients. This process will include multi-organ patients as well as single organ patients.
Collaborates with all team members to provide individualized, multidisciplinary patient care. Participates in quality improvement efforts in alignment with program, department, and hospital strategic plan. The Transplant Nurse Coordinator is a liaison and initial professional point of contact for providers and patients.
Manages and coordinates services at the UK Transplant & Specialty Clinic. Monitors clinic delays and trouble shoots problems real time to ensure smooth operations and exemplary customer service. Ensures adequate communication and cohesive teamwork occurs between the front office staff, the clinical staff and faculty. Works closely with Practice Manager and Transplant Administrator on operational issues as well as growth initiatives for the Transplant Clinic.
