ESSENTIAL DUTIES:

  • Record pertinent information pertaining to the client's care.

  • Remind clients to take medication at the appropriate time.

  • Plan and prepare meals according to specific dietary requirements of the client, and if necessary, feeding the client.

  • Assist the client with therapy exercises under the direction of a Registered Therapist.

  • Assist or perform personal care including hair care, oral hygiene, bathing, grooming, and dressing.

  • Assist with ambulation with or without mechanical aids.

  • Assist with routine bodily functions such as assisting clients with toileting.

  • Maintain a safe, clean and healthy environment through light housekeeping including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, providing and emptying bed pans and laundry.

  • Provide companionship and stimulation for the client including reading, walks, etc.; and accompanying the client to doctors or other appointments.

  • Turn or reposition patients who are bedridden.

  • Examine patient for bruises, blood in urine or other injuries/wounds.

  • Take client's vital signs and recording input and output.

  • Observe and report changes in client's condition to the Agency Director or Service Coordinator.

  • Accurately prepare daily records and submitting them to the office by date due .

  • Participates in in-service education programs.

  • Maintain a high degree of confidentiality at all times due to access to sensitive information.

  • Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.

  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements.

  • Abide by all regulations, policies, procedures and standards.

  • Perform other duties as assigned.

POSITION REQUIREMENTS & COMPETENCIES:

  • A Home Health Aide must have a minimum of one year of personal care experience which is verifiable through three work references, and must complete this agency's employment and competency process

  • Must be at least 18 years of age

  • Completion of high school or GED certification

  • Six months training as a Certified Nurse Aide and complete a competency evaluation

  • Must exhibit mature, responsible behavior and understand the need for patient confidentiality

  • Must be able to read, write and speak English

  • Must be able to follow direction and accurately report to the Administrator any changes in client's condition

  • Must have available reliable transportation to and from assignments. May be approved to transport client or run errands in vehicle

  • Must maintain current credentials, including CPR, First Aide, TB Test, Physical, Fingerprint Clearance Card, and attend required in-services or other requirements as mandated by the state or contract specifications

Nurse Care Manager I/II Work location: Remote position in Ohio. Must be within driving distance to one of the following Ohio offices: Cleveland, Columbus, Mason. Schedule: Monday through Friday, with some evenings required (up to 7m EST). The Nurse Care Manager performs care management activities within the scope of licensure for members with complex and chronic care needs. Responsible for collaborating with healthcare providers and/or consumer to drive personalized health management and improve health outcomes for optimal consumers. Performs care management activities within the scope of licensure for members with complex and chronic care needs. This role is primarily telephonic case management with the potential for on-site Case Management in the future. Candidate must be able to travel to Anthem offices or other provider locations, as needed. Primary duties may include, but are not limited to: * Ensures medically appropriate, high quality, cost effective care through assessing the medical necessity of inpatient admissions and extensions of stay, outpatient services, out of network services, and appropriateness of treatment setting and level of care. * Partners with physician clinical reviewers and/or medical directors to interpret appropriateness of care, intervention planning, and general clinical guidance. * Collaborates with providers to assess consumer needs for early identification of and proactive planning for discharge. * Conducts clinical assessment to develop goals that address individual needs in order to develop and implement a care plan. * Monitors and evaluates effectiveness of the care management plan and modifies as necessary. * Communicates effectively with wide array of clinical knowledge so as to educate members and empower them to manage their health care needs. Requires 3 years of acute care clinical experience; or any combination of education and experience, which would provide an equivalent background. * Current, unrestricted RN license for state of Ohiorequired. * Current, unrestricted RN license for state of Indiana, a plus, but not required. * Multi-state licensure is required if this individual is providing services in multiple states. * Home health/discharge planning experience preferred. * AS/BS in nursing preferred. * Certification as a Case Manager is preferred. Additional experience for the Nurse Case Manager II role: at least 1 year experience in a Nurse Care Manager I role or equivalent experience For URAC accredited areas, the following applies: Current and active RN license required in applicable state(s) that allows for an independent assessment to be conducted within their scope of practice. Requires 3 years full-time equivalent of direct clinical care experience to the consumer, 5 years full-time equivalent of direct clinical care experience to the consumer preferred or any combination of education and experience, which would provide an equivalent background, Multi-state licensure is required if this individual is providing services in multiple states. Certification as a Case Manager from the approved list of certifications or a BS in a health or human services related field also preferred. Please note: This position may be filled at either level.
