Duties:• Participate in and support efforts to coordinate and execute Antibiotic Stewardship Committee's goals and objectives in the hospital. • Perform the functions of a Pharmacist as defined in the Arizona State Board of Pharmacy Regulations. • Perform billing to Medicaid, Medicare, and all other 3rd party insurer patients. • Perform within the prescribed limits of the hospital's and Pharmacy Department's Policies and Procedures. • Responsible for reporting to the Pharmacist-in-Charge any observance of compliance discrepancies or issues. • Report appropriately through the chain of command. • Maintain strict patient and employee confidentiality at all times. • Maintain a Quality Improvement Program, The Joint Commission standards, Arizona State Statutes, and the SMH Quality/Risk Management program. • Maintain cooperative communication with other NHF/SMH departments to coordinate services to enhance patient care. • Provide clinical services during code blue and trauma situations. • Comply with and ensure all legal restrictions on the profession of pharmacy are followed. • Ascertain that drug usage is in accordance with formulary policies. • Work with Pharmacy automation system (Pyxis). • Interpret and enter/verify orders on inpatients and outpatient computer systems. • Monitor medication therapy and maintains medication records • Provide medication information to professional staff. • Dispense prescriptions and counsel patients. • Schedule medication, administrate and maintain MAR's. • Compound prescriptions and IV medications. • Order medication and supplies. • Check medication stock for expired or unusable stock. • Pick up and deliver medication and medication orders. • Clean and maintain pharmacy equipment. • Perform debits and credits to patient accounts. • Keep track of Controlled Substances within the hospital. • Respond to all codes as required for patient care in a timely manner. • Attend regular staff meetings to ensure communication among personnel regarding program-related activities. • Maintain professional and technical knowledge by conducting research, attending seminars, educational workshops, classes, and conferences. • Participate in Infection Control, Emergency Codes and Fire Safety Trainings. • Meet and comply with company Personnel Policies and Procedures. • Perform other duties as required.
Qualifications:Minimum Qualifications: • Doctorate's degree in Pharmacy. • Graduate of an ACPE accredited School of Pharmacy. • Three (3) years of progressive work experience in the healthcare industry or an equivalent combination of education and experience preferred. • Licensed by the State Board of Pharmacy in the State of Arizona as a Doctor of Pharmacy. • Must be able to successfully pass the Employee Health Program requirements and background investigation.
