SUMMARY: Pediatric advanced practice registered nurse (APRN) is to provide acute and primary care services to children from birth to young adults. A collaborative agreement is developed and updated through the credential process and as needed, with the current Physicians.

ESSENTIAL FUNCTIONS:

  • Utilizes advanced knowledge, skills, and expertise in pediatrics to implement an APN role for an identified population of patients, including acute & primary care, health promotion, prevention of disease, and management of chronic disease. Performs delegated medical functions as described in written protocols and/or a collaborative agreement that have been approved by medical and nursing

  • Systematically assesses the physical, psychosocial, and developmental needs of patients and families through the use of a medical screening exam (MSE), history and physical exam, and other assessments as Develops a plan of care for each patient/family based on assessment and documents according to policy.

  • Demonstrates advanced nursing clinical judgment and competence consistent with evidence-based practice in assessment, planning, implementation, and evaluation of identified patients. Monitors clinical outcomes to improve the quality of care for patients. Evaluates and modifies the plan of care as needed.

  • Facilitates collaborative practice with the multidisciplinary team to provide a comprehensive, coordinated needs assessment and treatment plan. Communicates the plan to the patient, family, and other care providers, both within the Hospital and with appropriate community agencies to provide continuity of care. Acts as a patient advocate.

  • Integrates the role of teacher into clinical practice to provide patient education. Assesses learning needs, provides for education and evaluates learning objectives are achieved. Assists the staff in providing patient and family education.

  • Promotes professional growth and development of staff nurses by providing continuing education to advance knowledge and skills in identified areas of clinical expertise. Provides leadership by assisting new practitioners and graduate nursing students with role development and socialization. Interprets scientific information in areas of expertise through formal and informal educational

  • Consults within the organization on current pediatric clinical practice and disease management in area(s) of expertise. Functions as a resource to the community in areas of

  • Participates as a member of related professional organizations.

  • Functions as a role model and informal leader.

  • Demonstrates competence in nursing research through:

  • Utilization of research studies to guide clinical practice

  • Participation in research studies

  • Evaluation of outcomes

  • Interprets and communicates research findings to nursing staff to advance

ADDITIONAL FUNCTIONS as needed:

  • Provides acute care and follow-up to patients in the ACC and WIC.

  • Provides well-child care in conjunction with the primary care

  • Provides services related to health maintenance and promotion.

  • Establishes diagnoses for common short-term and chronic health

  • Orders and conducts timely follow-up on laboratory and radiologic

  • Performs other therapeutic or corrective measures as

  • Prescribes medications as

  • Performs medical screening exams for patients that present to the acute care clinic and provides urgent care as indicated.

  • Consults and/or refers patients to licensed physicians or other health care providers as

  • Reviews laboratory results, consultant examinations and emergency room dispositions and follow up on those requiring

  • Appropriately documents all findings concisely and completely utilizing the electronic medical records system, ensuring all entries are dated and signed within 48 hours of the patient

  • Participates in special projects and quality initiatives as assigned by Chief Medical Officer, Chief Nursing Officer, and Outpatient Executive

  • Stays current with evidence-based practice and standards of practice.

  • Educates patients on the appropriate use of care services including the WIC and ACC and develops materials supporting increased patient knowledge in this

  • Serves as a resource and consultant for other providers, nurses, and staff members within the

  • Assists in the education of patients, nurses, and other clinical team members.

  • Performs other duties as assigned.

Job Requirements:

QUALIFICATIONS:

Education: M.S Degree; training as a Family Nurse Practitioner or Pediatric Nurse Practitioner

License: Current Illinois Advanced Practice Nurse (APRN)

Experience: 3 years or more experience as an APRN; pediatrics preferred

Job Type: Full-time (1.0 FTE)

Language: Bi-lingual Spanish preferred

Specialized Knowledge and Skills: Excellent written and verbal communication skills, excellent interpersonal skills,

demonstrate knowledge and expertise in patient care (pediatrics preferred). Active

member in appropriate professional organization

Certifications: CPR issued by the American Heart Association or the American Red Cross;

Pediatric Advance Life Support preferred
