Sr Social Worker (Family Med/Transmountain)

Position Description

The case manager serves as counselor and resource person for a caseload of patients. Specific duties include: providing individual and group counseling; participating as a member of a treatment team; corresponding with outside agencies, institutions and families of patients; collecting and writing social histories; monitoring patient/client treatment plan reviews; and compiling staffing notes. Functions under the immediate supervision of a supervising psychologist, social worker, or physician.

Preferred Qualifications

• Masters in Social Work

• Minimum of 2 years of family therapy or counseling experience with a diverse clientele covering the age spectrum (pediatrics to geriatrics)

• Previous experience with educational responsibilities for health profession learners including writing behavioral health curriculum, working with preceptors, and presenting at educational conferences is highly preferred

• Excellent writing and verbal skills

• Has the ability to multitask and prioritize and work in a multi-disciplinary team

• Previous clinical or medical social work experience in a medical setting is highly preferred

• LCSW or current eligibility and completed licensure in the state of Texas within 6 months

• Bilingual in English and Spanish

Required Qualifications

• Bachelors degree in Social Work and 4 years of related experience OR Masters degree in Social work and 2 years of related experience

• Credentialed as a Certified Social Worker
