New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

As a Sr. Mental Health Practitioner your responsibilities will include:

  • Provides person-centered case management for an assigned group of FACT members including coordinating and monitoring the activities of the individual treatment team (ITT)

  • Participates in comprehensive assessment of psychiatric history

  • Performs shift management in coordination with other FACT shift managers according to established policy and procedures

  • Provides on-going assessment of members' mental illness symptoms and members' response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put members at risk (e.g., suicidality)

  • Provides symptom education to enable members to identify their mental illness symptoms

  • Provides multiple-stage individual and group treatment in the office and in community settings to develop a trusting relationship with members so that they may more freely discuss substance use and its effect on mental and physical health and daily functioning

  • Plans and provides work-related supportive services, such as assistance with grooming and personal hygiene, securing of appropriate clothing, wake-up calls, and transportation

  • Ensure that members have adequate financial support (e.g., help to gain employment or apply for entitlements)

  • Organize and lead individual and group social and recreational activities to structure members' time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support

Requirements

New Horizons will need you to have the following qualifications:

  • Master's Degree in Social Work or Counseling is preferred

  • Two (2) years' experience working with children/adolescents

  • CAP certification required
