Job Summary

In this Senior Level position, human services experienced staff are responsible for the seamless integration of wrap around practices and managed-care methodologies to ensure that services implementation and care coordination are available to all new referred families to the Community Service Agency network.

What You Will Be Doing to Make a Difference in the Lives of Families...

Wraparound Team Management

  • Provide weekly individual supervision to Intensive Care Coordinators on staff; all supervision must be documented in files accessible for review by the MCE during the site review process or upon request

  • Supervision notes must contain, at a minimum, information regarding frequency of supervision, format of supervision, supervisors' credentials, and general content of supervision sessions

  • Conduct comprehensive, home based assessment inclusive of CANS and other tools as determined necessary, which occurs in youth's home or other location of family's choice

  • Develop risk management/safety plan in collaboration with the family and collaterals

  • Maintain regular contact with the family, youth (where appropriate) and other relevant persons in the youth's life

  • Facilitate CPT meetings

  • Maintain face to face contact with the youth and family, as determined by the youth, family, and members of the CPT

  • Make necessary referrals and linkages to appropriate supports as identified in the CPT

  • Identify and develop with the family natural supports

  • Assist in system navigation

  • Provide family education, advocacy, and support

  • Identify and actively assist youth and family to obtain and monitor delivery of available resources including medical, educational, social, therapeutic, and other services

  • Monitor, review and update the ICP to reflect the changing needs of the youth

Quality Management

  • Meet productivity expectations

  • Facilitate TOMS on staff

  • Maintain policies and procedures for safety and well-being that are consistent with the standards of Bay State Community Services

  • Maintain standards of confidentiality as defined by Bay State Community Services

  • Participate in Area CQI initiatives and CSA Program self-assessment as defined by Bay State and Mass Health

  • Participate in Performance Improvement Planning based on aggregate data to maximize utilization and efficient services while supporting family access and family inclusion in Care planning activities

Professional Growth and Development

  • Participate in supervision for professional growth, one hour per week individual and one hour peer week group by a clinician licensed at an independent level

  • Attend all mandated trainings offered by Bay State, Mass Health or TAC

  • Participate and promote culturally relevant practice

  • Engage in annual self-evaluation to establish goals for professional development

Requirements

Qualifications/Credentials

  • Master's degree in Human Service Field with at least 3 years of experience in providing outpatient behavioral health services to youth and families

  • Must hold and maintain valid independent MA state professional license (LMHC, LMFT, LICSW, etc.)

  • Experience with home based or wrap around models is preferred

  • Supervisory experience preferred

  • Must have experience working collaboratively with state agencies, consumer advocacy groups, and or behavioral health outpatient facilities

  • Must meet credentialing criteria for Master's level Clinicians as outlined in the MCE's most current provider manual

  • Experience in navigating the child/family serving systems and experience advocating for family members who are involved with behavioral health systems

  • MA CANS eligible

  • Valid MA driver's license with acceptable driving record and current proof of insurance

  • Acceptable CORI and SORI background record check as required by program
