The Spiritual Care Coordinator is a member of the Hospice Team who coordinators and provides spiritual care for hospice patients and their caregivers.
This is a part time, 20-hour position. Your responsibilities will include:
Provide direct counseling services to hospice patients and the bereaved
Is effective in spiritual counseling of individuals regardless of their belief system, values and culture factors.
Develops and coordinates communication with local clergy
Assesses the spiritual needs of the hospice patient and family
Documents visits according to agency policies
Participates as a member of the Hospice Team is accessing, implementing and re-evaluating hospice care plans
Perform other duties assigned by supervisor
Clinical Pastoral Care, Masters preferred in the Ministry or Medical Social Work.
Possesses and demonstrates organizational and interpersonal skills and is knowledgeable about community resources and bereavement issues.
Must plan appropriately for in-service, group sessions and memorial services.
