The San Mateo Medical Center is seeking an experienced Spiritual Care Coordinator

  • Extra-Help. The Spiritual Care Coordinator, as a member of the interdisciplinary treatment team at San Mateo Medical Center, provides spiritual, non-denominational, and inclusive support and services to patients, their families, and staff. The Spiritual Care Coordinator provides spiritual encouragement, hope, healing, and communication with patients as part of comprehensive, integrative patient care. Services and resources provided by the Spiritual Care Coordinator acknowledge, respect, and guard each patient's beliefs and practices as an integral part of the patient's rehabilitation and maintenance of overall health.

Theideal candidate will:

  • Possess knowledge of:

  • Clinical needs of varying patient populations (medical/surgical, emergency room, psychiatric, infusion center, intensive care, long term care/rehabilitation, geriatrics, community health/outpatient).

  • Broad variety of spiritual/religious and cultural practices/beliefs of diverse patient populations served within the San Mateo County Health.

  • Resources in other languages that can help provide spiritual resources/support to patients.

  • Professional and ethical codes of practice (as outlined by the Association of Professional Chaplains and/or training in Clinical Pastoral Education).

Adhere to ethical and professional codes of practice, as outlined by the Association of Professional Chaplains and/or training in Clinical Pastoral Education and assume personal responsibility for work done with patients.Demonstrate compassion for patients, family members, and others.Possess ability to handle emotionally stressful situations.Display good verbal and written communication skills and be able to professionally receive and follow oral instructions and feedback.Keep records/data of interactions with patients, as well as monthly statistics.Possess a Bachelor's or Master's Degree in Theology or closely related field (ex. Divinity, Religious Studies, Pastoral Ministry, Pastoral Studies, Spirituality), required.Previous experience in hospital or healthcare setting preferred.Experience in developing and managing volunteers preferred.Ability to speak, read, write in Spanish preferred.The current vacancy is a Full-time, Extra-Help position. This is a short-term assignment, and the work hours of this position are Monday

  • Friday, 8 am- 5 pm.

NOTE:This is an extra-help position, at-will assignment, paid on an hourly basis. Extra-help positions are short-term employment for up to 1040 hours. Some extra help positions are eligible for benefits under the Affordable Care Act. Extra help employees are not guaranteed permanent status at the end of the assignment.

Examples Of Duties

Duties may include, but are not limited, the following:

  • Provide spiritual support and resources to patients, families, staff, and physicians by responding to referrals, codes, and requests for spiritual care. Services can be non-denominational and/or faith/religion specific, if indicated and/or requested by patient, patient's family, and interdisciplinary treatment team.

  • Provide above mentioned spiritual support/resources to all needed inpatient areas/units of SMMC (acute medical/surgical, intensive care, emergency room, inpatient psychiatric, infusion center, long term care/skilled nursing). Provide mentioned services as needed to other areas of the San Mateo County Health (ex. Aging and Adult Services, etc.)

  • Provide spiritual support and resources on-call and after hours as needed.

  • Provide information and assistance on patient Advance Directives.

  • Gather and evaluate relevant data pertinent to the patient's situation and/or health and well-being.

  • Assess patients' spiritual needs using the department's spiritual assessment tool. Based on assessment, provides appropriate and specific spiritual and emotional support for the needs and issues identified.

  • Document the spiritual assessment and spiritual care/treatment plan needs, as well as chart follow-up progress notes.

  • Respect the confidentiality of information from all sources in accordance with HIPPA laws/regulations.

  • Attend interdisciplinary team and patient/family meetings regarding patient's overall progress/treatment and contribute to team meetings the spiritual component of the overall patient care.

  • Act as a liaison with other agencies and resources out in the community providing spiritual resources/services.

  • Collaborate with community agencies/resources, including agencies within San Mateo County Health such as Aging and Adult Services, to provide a consistent support network and continuity of care for patients who are followed both in the hospital and out in the community post discharge.

  • Develop and oversee the Spiritual Care Services Program at SMMC, including overseeing existing and potential new spiritual/pastoral volunteers that provide visits and resources. For some units, coordinate spiritual/pastoral volunteer services in order to provide regularly scheduled on-site services for patients as part of Title 22 mandated therapeutic programming requirements, such as long term care.

  • Assume responsibility for continued professional development, demonstrate a working and current knowledge of current theory related to spiritual care services.

Qualifications

Knowledge of:

  • Principles, practices, and administrative requirements of the program to which assigned.

  • Contemporary social, political and economic trends and problems related to community services.

  • Principles of social planning and program development related to community services.

  • Specialized technical knowledge related to assigned program area.

  • Monitoring and evaluating systems for community services programs.

  • Formulation and function of community and volunteer organizations.

  • Current social and community needs.

  • Principles and practices in community development and services planning.

  • Methods applied to the collection and evaluation of statistical data.

  • Principles of community organization.

  • Applicable federal, state and local laws, rules and regulations and County and program policies and guidelines.

  • Computer applications related to the work.

  • Office administrative practices and procedures, including records management and the operation of standard office equipment.

Skill/Ability to:

  • Make independent judgments and work independently.

  • Prepare concise reports and recommendations.

  • Communicate effectively in writing and orally.

  • Coordinate multiple facets of a program function.

  • Interpret rules, regulations and guidelines at all government levels.

  • Compile, analyze and interpret data using statistical and research techniques.

  • Establish and maintain cooperative working relationships with members of other departments and diverse community groups.

  • Plan, organize, promote and coordinate diverse community program activities.

Education and Experience:

Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying. A typical way to qualify is:

Community Program SpecialistI is the entry and first working level class in the Community Program Specialist series with employees performing work related to program planning, coordination and evaluation, contract monitoring, research and reporting. Work may be limited to one specialized area, or it may encompass several areas. This class is flexibly staffed with Community Program Specialist II and incumbents may advance to the higher level after gaining experience and demonstrating proficiency, which meet the qualifications of the higher-level class.

Community Program Specialist II: Four years of experience in an organization providing social services which has included responsibility for community service program development such as planning, evaluating, monitoring or coordinating projects.
