  • Possess a Clinical or Rehabilitative Services Credential with authorization language in Speech and Hearing *Interns Eligible (Must attach letter of eligibility) Application will be considered incomplete without the following attachments: *Appropriate credential *Three (3) letters of recommendation (signed and dated within two years of date of application) *Letter of Introduction/Interest *Current Resume Do not attach a driver's license or any document with a photo of yourself and/or indicating your age, your application will be automatically rejected and screened out.
