Job Description and Responsibilities

Willow Valley Communities, NOT a For-Profit Therapy Company that has a wonderful culture and beautiful surroundings is looking for a Speech-Language Pathologist (PRN). This Speech-Language Pathologist (PRN) plans, organizes and administers medically prescribed Speech Therapy programming for patients with speech and language communication disorders, cognition and memory decline, swallowing difficulties, to improve communication skills, increase cognition and memory recall, ensure safe diet levels and improve quality of living.

Wage:

  • Pool/PRN $55/hr

Responsibilities:

  • Models exemplary customer service and models care, compassion and trust toward Team Members and Residents.

  • Diagnoses and evaluates speech and language skills, cognitive, and swallowing skills as related to medical, social, and psychological factors.

  • Reviews Physician's referral (prescription), patient's condition and medical records to determine speech/dysphagia therapy treatment required.

  • Provides counseling, guidance and language development therapy to disabled individuals, caregivers and family.

  • Reviews individual chart to obtain background information prior to evaluation in order to determine appropriate tests and to ensure that adequate information is available.

  • Administers, scores and interprets assessments for speech, language, cognition.

  • Tests and measures patient's swallowing function.

  • Plans and prepares written plans of treatment based on evaluation data to restore swallowing, communication, and cognitive function.

  • Observes, reports, and records treatments, responses, and progress in patient's chart.

  • Reviews treatment plan, and assesses individual performance to modify, change, or write new programs.

  • Instructs individuals and/or caregivers to monitor their own speech and swallowing, and provides ways to practice new skills.

  • Attends care plan meetings as appropriate for interdisciplinary team updates.

  • Conducts in-services to staff in speech therapy techniques and objectives.

  • Confers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate occupational therapy treatment with other aspects of patient's health care.

  • Ability to work with other therapists, nursing staff, patients, and caregivers in coordinating patient care to optimize outcomes.

  • Requisitions supplies and equipment.

  • Acts as consultant to medical and other professional groups.

  • Attends meetings, seminars and participates in other activities to promote professional growth.

  • Ability to work with other therapists, nursing staff, patients, and caregivers in coordinating patient care to optimize outcomes.

  • Demonstrates good organizational, time management, and planning skills.

  • Demonstrates good working knowledge of Prospective Payment Systems (PPS), Minimum Data Set (MDS), Medicare Part B, and other applicable reimbursement systems.

  • Adheres to all applicable federal, state and local standards of care and the ethics and policies concerned with the practice of speech therapy.

  • Completes timely clinical documentation to include functional goal-setting, programming and therapeutic approach.

  • Completes billing/administrative documentation timely and accurately.

  • Completes Resident screens in conjunction with annual MDS schedule and in response to nursing reports/referrals.

Qualifications:

  • Master's degree from accredited speech-language pathology program at a 5 year college or university.

  • Must be board certified and licensed by the state.

  • Computer literacy is required.

  • PRN/Pool (as needed)
