Seeking full time 30 hour/week SLP / Speech Language Pathologist for our skilled nursing facility located in Monroe, NC.

The primary purpose of the position is providing and directing speech therapy services to patients which may include pediatric, adult and/or geriatric persons. The goal of helping them reach their maximum performance potential within a medical model of practice shall occur.

Responsibilities

  • On referral, evaluate the impact of available resident information on the assessment and treatment process.

  • Conduct and document a thorough assessment of the resident to determine and diagnose communicative deficits and equipment needs.

  • Develop and implement an individualized treatment program based on the initial evaluation and containing achievable functional goals.

  • Comply with evaluation, treatment and documentation guidelines, and consistently maintain timely and accurate departmental records.

  • Instruct aides in appropriate procedures and provide supervision in accordance with applicable State law.

  • Enter a progress note in the resident record following each treatment session.

  • Revise the resident's treatment program, as needed, based upon resident performance.

  • Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format.

  • Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedure.

  • Work with nursing staff, resident and families to maximize resident's use of functional abilities.

  • Instruct residents and family/other caregiver about therapeutic procedures to be continued by the resident at home following discharge.

  • Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources.

  • Attend and participate, or provide information for rehabilitation team conferences regarding resident progress, problems or needs.

  • Attend and participate in other department/facility meetings, as required.

  • Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency.

  • Participate in and/or provide in-service education sessions.

  • Participate in the quality improvement process by responding appropriately to results of medical record audits, resident satisfaction surveys, safety activities and regulations.

  • Promote the programs and services of Heritage Healthcare through formal and informal interactions with the community.

  • Act in compliance with Heritage Healthcare's regulatory and professional standards and guidelines.

  • Adhere to facility policies and procedures and participates in facility quality improvement and safety programs.

  • Perform other duties as assigned and required to provide quality care to residents.

  • Understand reimbursement by various payers and allocate therapy resources efficiently in accordance with resident goals and outcomes related to discharge destination.

  • Demonstrate a knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.

  • Support the company's commitment to Compassionate Care; project a professional image to foster confidence in the Rehabilitation Program.

  • Perform duties in an independent manner and recognizes situations where direct supervision is needed; willingly helping others.

  • Maintain the expected productivity level that has been discussed and is expected for your position at your particular facility/facilities.

Qualifications

  • Master's Degree in Speech and Language Therapy.

  • Hold a current Certificate of Clinical Competence in Speech- Language Pathology (CCC-SLP) credential offered by the American Speech-Language-Hearing Association.

  • Assumes responsibility for ongoing continuing education and professional development

  • Must maintain valid state Speech Language Pathology licensure

VIEW JOBS 3/17/2019 12:00:00 AM 2019-06-15T00:00 General Statement The Foreign Language Interpreter, under general supervision, assists the Department of Human Services to expand the capacity to deliver effective services to Limited English Proficient clients. This is accomplished by the help the interpreter provides for the communication between Human Services employees and the clients. Employees can provide interpreting services to the divisions within the Department of Human Services and may occasionally be asked to interpret calls that come through the main government center switchboard. Essential Functions Conveys communication between the provider and client, while converting the exchange from one language into another. The interpreter works in all program areas and in a variety of settings including homes and community sites. Interpreters act as accurately as possible, given the idiomatic difference between languages, while keeping the spirit and letter of the original communication. Communicates possible misunderstandings by the client to the provider. Assists the provided in understanding the client's culture. This included nonverbal communications, health practices, and social relationships. The interpreter does not attempt to give explanations other than what is communicated by the professional. Maintains strict confidentiality regarding any and all client-provider information. Refrains from inserting own opinion or judgment when communicating with clients. Provides translation of written letters and simple written informational and instructional material, being very careful to translate as literally as possible, given the idiomatic differences between languages. Performs work during emergency/disaster situations. Performs clerical and other related duties as assigned. Has knowledge of office practices and procedures. Performs under stress and when confronted with persons acting under stress. Maintains high standards of accuracy in exercising duties and responsibilities. Communicates clearly and effectively in person and by phone; uses tact, courtesy, and decorum to project a favorable image for the agency. Additional essential functions based on the assigned department: Public Health Assists health care professionals with medical exams to include setting up exam rooms, obtaining and recording patient vital signs, performing vision and hearing tests, acting as a chaperone during exams as needed, demonstrating proper handling of specimens. Maintains clinic supplies to include monitoring supply levels and stocking rooms with supplies. Minimum Qualifications Education and Experience: Requires a High School Diploma or an acceptable equivalency diploma (GED), and five (5) years of experience as a translator/interpreter; or equivalent combination of education and experience. Licenses or Certifications: Public Health Medical Office Assistant Certification or Certified Nursing Assistant Certification. Successful completion of Union County Public Health clinical assistant training (provided upon hire).
