We are currently seeking a Pediatric Speech Language Pathologist (SLP) for a school-based position:

School-Based Speech-Language Pathologist (SLP) Responsibilities:

  • Implement specialized therapy treatment, techniques, and instruction in accordance with Individualized Education Programs (IEPs) of students being served
  • Screen, evaluate, and write reports on student levels, strengths, needs, and progress using approved testing and assessment instruments
  • Participate in Individualized Education Program (IEP) and multi-disciplinary team meetings to evaluate student needs/progress and to develop and/or revise individualized therapy interventions, objectives, and goals
  • Collaborate with teaching staff to plan and implement classroom-based activities and instructional techniques to ensure carry-over of learning
  • Provide consultative services to parents in the therapy program of their child
  • Compile, maintain, and file all required student documentation and reports in compliance with deadlines, confidentiality laws, federal and state laws, State Board of Education regulations, and school administrative policies and procedures
  • Maintain up-to-date knowledge of research, theories, and practices associated with school-based therapy

School-Based Pediatric SLP Qualifications:

  • Valid state license as a provider of Speech-Language Pathology
  • Pennsylvania Department of Education (PDE) Teacher Certification
  • Child Abuse, Federal, and State Clearances

VIEW JOBS 6/24/2022 12:00:00 AM 2022-09-22T00:00 <p>Access Services is hiring two <strong>Community Based Case Manage</strong><strong>rs</strong> for its <strong>New Life Youth and Family Services </strong>program. This role is based out of a <strong>West Chester, PA</strong> office and serves the <strong>Chester County</strong> community and neighboring counties. </p><p>The Community Based Case Manager will conduct a thorough assessment of cases assigned, and in consultation with parent/guardian, child/adolescent, school/agency, and the Supervisor of Community Based Services, examine and identify the factors contributing to the child/adolescent’s identified issues and needs. This consultation will include but not be limited to phone conversations, review of records, and an initial meeting, which would involve the child/adolescent, their parents/caregivers, and/or school personnel and other agencies. The Case Manager will formulate an intervention plan specifically designed to address the child/adolescent and their needs. The Case Manager will then provide services to accomplish the intervention plan and coordinate all referrals to community services as appropriate. </p><p><strong>Work Schedule: </strong> Full time (40+ hours), Monday-Friday, 9am-5pm with some flexibility to meet client needs.</p><p><strong>Requirements</strong></p><p><strong>Key Responsibilities of Community Based Care Manager: </strong></p> <p>1. Assess strengths and needs of child and family with referring agency, the Department of Children, Youth and Families (DCYF).</p> <p>2. Determine, in conjunction with the Supervisor of Community Based Services or the Executive Director of Community Based Services, child and family, school/agency, and/or county child welfare caseworker/juvenile probation officer, appropriate services.</p> <p>3. Provide child-centered, family-focused, individual, family, or group services. Services will be conducted in the home, school, or other community settings.</p> <p>4. Establish and maintain relationship with families, schools, and other providers. </p> <p>5. Maintain regular communication with the referral source.</p> <p>6. Communicate regularly with outside agencies that provide services to the child and their family, monitoring client/family progress.</p> <p>7. Participate in interagency meetings pertaining to the child and family.</p> <p>8. Facilitate a comprehensive written assessment, goal plan, goal plan review, monthly reports, court reports, daily progress notes, and discharge summary for each child, under the direction of the Supervisor of Community Based Services.</p><p><strong>Requirements and Qualifications</strong></p><p><strong>Education: </strong>Bachelor’s degree in social work or related field required.
