PRN, pay per visit home health Speech Therapy position. Service area includes Robertson, Cheatham and Montgomery counties.

Definition:

The Speech-Language Pathologist supervises and participates in the application of therapeutic techniques for the rehabilitation of patients with speech and language disorders, hearing disorders and oral myofunctional disorders.

Line of Authority:

Director of Therapy Services, Administrator/Director of Services

Qualifications:

  • Must have a master's degree from an approved curriculum in Speech Pathology, or its equivalent.

  • Must have a Certificate of Clinical Competence (CCC-SP) from the American Speech and Hearing Association or be eligible for a Clinical Fellowship Year (CFY).

  • Must be licensed in the state or states in which he/she will work.

Performance Requirements:

  • Able to transfer, lift and guard patients safely with appropriate assistance.

  • Able to bend, stoop, squat and lift numerous times daily (70 80 lbs) in the care of patients and use of equipment.

  • Able to carry out fine motor skills with manual dexterity.

  • Mental acuity and judgment skills to adequately and safely perform job requirements.

  • Able to see and hear adequately in order to respond to auditory and visual needs and clinical indications of the patients.

  • Able to learn, absorb and apply professional training.

  • Able to organize work procedures, assume responsibility and tactfully supervise others.

  • Able to speak in a clear, concise voice in order to communicate adequately.

  • Able to read, write and edit documentation.

  • Must have reliable personal transportation.

Specific Responsibilities:

  • Directs and supervises the Communication Disorders Service Department.

  • Provides evaluations and therapy as ordered by the referring physician and recommends an appropriate treatment program for the physician's approval.

  • Evaluates, plans, and carries out therapy programs, including the establishment of goals for patients as referred and approved by the physician, with periodic re-evaluations.

  • Participates in the in-service training program for the HomeCare personnel.

  • Consults with the HomeCare staff concerning communication problems of patients not receiving speech pathology services.

  • Conducts or contributes to speech pathology related CQI studies as requested..

  • Maintains patient care records as designated by the HomeCare agency assuring that third party reimbursement qualifications are met, as well as all state/federal requirements.

  • Consults with family members, physicians and other health care professionals as appropriate.

  • Participates in interdisciplinary conferences to maintain continuity of patient care.

  • Serves as Utilization Review and/or Advisory Board Member as requested by the Administrator/Director.

  • Maintains knowledge base of HomeCare Speech-Language Pathology procedures and regulations as well as professional standards through attendance at workshops, in-services and individual research and reading.

Essential Functions * Uses a chainsaw to cut brush, logs and stumps of larger trees. * Gathers brush and feed brush into wood chipper. * Applies chemical herbicide to right-of-way, following brush removal * Utilizes hose and spray nozzle from truck mounted spray equipment which includes tank, pump, hose reel, 200 feet of ½ inch hose, or utilizes back-pack to treat remote areas. * Services truck, chipper, and equipment, maintaining them in neat, clean, and specified mechanical order. * Assists senior spray person in mixing herbicide in holding tank. * Works around hazardous equipment. * Works around excessive noise from machines, chain saws, wood chipper, with hearing protection. * Works in all temperatures and weather and will be exposed to nature, i.e., irritating plants, biting or stinging insects, dust, etc. * Works on unlevel ground. * Works around falling limbs with protection. Minimum Requirements * Due to the inherently dangerous nature of the industry and requirements to work with or around hazardous equipment, employees must be able to maintain attention and concentration for extended periods of time. * Must be able to wear necessary PPE as necessary. * Must be able to enter and exit a vehicle numerous times a day. * Must be able to withstand exposure to all kinds of weather while completing work assignments, i.e., rain, heat, sun, cold. * Must be able to traverse various terrains. * Must be able to write, read and comprehend written and verbal job instructions/information. * Must be able to communicate with others. * Must be able to maintain attention during dangerous tasks. * Must be able to lift, move and maneuver up to 50 pounds on an infrequent basis, and up to 10 pounds on a frequent basis. * Must have endurance necessary to perform these duties throughout a standard eight or ten hour day. Must be able to work long hours and overtime during emergencies (including holidays, weekends, etc. Subject to call out work. * Must have sufficient eyesight to judge distances and coordination to get and use the appropriate equipment and tools. * Must be able to operate and service all required tools and equipment. * Must have transportation to and from the startup location. * Must have a valid driver's license. * Must pass a background and/or MVR check. * Must pass a pre-employment drug screen.
