Education Requirements Degree from accredited College of Pharmacy. Must be registered or eligible for registration in this state.

Membership in professional organizations is desirable. A residency or fellowship is desired. Description

A. Provide Pharmacotherapy for all covered patients.

B. Take Call per Call Schedule (code response and on call coverage) C. Precept students, residents and mentor pharmacists. D. Complete all departmental documentation in timely manner.

E. Participate in all aspects of formulary management. F. Develop staff development programs, performance improvement projects, and policy and procedure. G. Participate actively in designated committees. H. Provide positive role model for department.

Position Requirements Must possess experience that allows the individual to effectively interact and positively relate to members of the medical, nursing and pharmacy staffs. Must type accurately and with moderate speed.

Must maintain a neat, clean, and orderly appearance at all times. Must maintain a calm attitude and mature outlook in moments of stress. Must have a strong personal ethics and impeccable professional conduct. Must possess strong personal interrelationship skills, exceptional communication and organizational skills and a high tolerance level.

Hospital experience is desirable, but On-the-job training is provided since many tasks and methods are indigenous to hospital pharmacy. Description Supervises the dispensing of all medication to provide the correct medication in the correct strength and dosage form to the correct patient in a safe and timely manner while being consistent with professional judgment, hospital policies and the laws governing the practice of pharmacy. Safeguards the patient by screening their drug regimen for allergies, drug interactions, proper drug and proper dosing and therapeutic duplication. Provides drug information to allied health professionals and the patient. Performs special assignments as directed by the Director of Pharmacy or other pharmacist supervisors.
