ATTENDANCE REQUIRED FOR THIS POSITION ARE AS FOLLOWS: Non rotating 40 hour work week, Monday

• Friday, 8:00 am

• 5:00 pm. Must be available to work additional hours as needed.

Responsibilities

Under the direct supervision of the Nutrition Program Coordinator. Performs the essential duties of an Outreach Worker.

Receives, logs, screens, and processes requests for assistance from the community. Conducts home visits to assess client needs and eligibility for the Home Delivered Meal Program. Develops case plans and facilitates services for clients with outside social service agencies and makes appropriate referrals.

Monitors client's progress and conducts regular reassessments of client eligibility status daily. Collects, compiles and submits required reports on a daily basis to the Nutrition Program Coordinator. Provides a full array of direct social services on a one to one basis with the elderly community.

Identifies isolated, low-income, minority elderly to determine eligibility for benefits and services they might otherwise not access. Displays good working knowledge of community resources and social service agencies. Responds to crisis situations and provide assistance to seniors in need.

Communicates with route drivers on any significant changes in client status. Works closely with the Nutrition Program Staff to coordinate an efficient delivery route system. Adheres to HDM program standards governing operations.

Must have use of personal vehicle which meets State minimum insurance requirements and possess a TX Driver's License. Must obtain Food Manager's Certification within 90 days of employment and Basic First Aid and CPR within 90 days of employment. Benefits Counselor Certification

• Level II within 180 days of employment.

Will be asked to perform other duties as assigned.

Qualifications

Graduation from high school or G.E.D. with a minimum of two (2) years of college training (60 hours) in a related field such as psychology, or social services. Two years of full time paid experience in direct social services including with a minimum of one year of experience dealing directly with the elderly.

One year of college training may be substituted for one year of general experience of any equivalent combination of training and experience that is related to the essential functions of the position. Bilingual ability preferred.

Basis of Rating

Application review and the City may also conduct additional skill assessment tests, in addition to the panel interview.

Selected applicants must be able to pass a background investigation and a pre-employment drug test.
