Position Overview

Seeking a Licensed Clinical Social Worker to provide Hospice care for the Pediatric Team in the Rockford, McHenry, Elgin, Schaumburg, and Barrington Areas. This Community based Pediatric position is designed to reduce hospital readmissions, focus on patients and their goals of care. This Social Worker will provide direct casework, counseling services and psychosocial assessments to Hospice patients and families. This person will play an integral role and active participant in Hospice interdisciplinary team along with working closely in connecting families with services to meet their needs.

Experience and Skills:

Job Requirements

  • Master of Social Worker degree required
  • LCSW Licensure in Illinois required

Preferred Experience, Skills and Qualifications

  • Prior experience in medical setting working with pediatric patients; hospice and/or home care experience preferred

  • Access to a reliable car, current driver's license and liability auto insurance

Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives. # Education: Bachelors degree in Social Services Field (Human and Family Resources, Social Work, Sociology or Psychology) or related discipline. Experience/Skills: Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Good interpersonal skills and ability to work with patients, physicians, families, managers and members of the community under varied conditions. Excellent organizational, critical-thinking and conflict resolution skills required. Physical Requirements: Ability to lift up to 50 pounds with frequent standing and walking.
